Mission

The inn between is utah's only end of life and medical respite housing solution forterminally ill and medically compromised adults experiencing homelessness. Theprogram opened in august 2015 to address a critical gap in homeless services. The inn between's mission is to end the tragic history of vulnerable people dying on thestreets of our community. They achieve this by providing safe and supportive housing for people who have nowhere to live during a medical crisis. They envision a community where people take care of each other, especially those less fortunate, and where everyone has a safe place in which to 1) experience the end of life with dignity and with access to professional hospice care (a critical medical service that cannot be delivered in shelters, motels or encampments); 2) recuperate from acute medical illness, injury, or surgery; or 3) to gain access to life-saving medical interventions that require stable housing (such as cancer treatment.for terminally ill individuals

The inn between's conflict of interest policy establishes the procedures applicable to the identification and resolution of conflicts of interest in the context of transactions or arrangements entered into by the inn between where an interested person may have a financial interest (defined below) in or fiduciary responsibility toward an individual or entity with which the inn between is negotiating a transaction or arrangement. The conflict of interest policy is routinely and systematically used to prevent actual, potential, or perceived conflicts of interest that might negatively impact the inn between, the donors who support the inn between, or the inn between's irs designation as a 501(c)(3) nonprofit. After being made aware of a potential conflict, the executive director may determine whether a conflict of interest exists for an employee or volunteer, and may determine the appropriate response, with the help of the executive board as needed. The executive board shall determine whether a conflict of interest exists for the executive director or a member of the board and shall determine the appropriate response. The interested person may make a factual presentation to the executive director or executive board and may answer questions, after which time, he or she shall leave the meeting to allow the executive director or committee an opportunity to discuss the situation and to vote on whether or not a conflict of interest exists, and if so, what steps should be taken to remediate the conflict. The interested person shall not actively participate in the discussion of or vote on the transaction or arrangement, either formally at a board or committee meeting or informally through contact with individual board or committee members. In addition, the interested person should not be counted in determining whether a quorum is present for the board or committee meeting at which the transaction or arrangement that results in the conflict of interest is to be voted upon. The executive director or board chair shall, if appropriate, appoint a disinterested person or committee to investigate alternatives to the proposed transaction or arrangement. After exercising due diligence, the executive director, board chair or committee shall determine whether the inn between can obtain a more advantageous transaction or arrangement with reasonable efforts from a person or entity that would not give rise to a conflict of interest. If a more advantageous transaction or arrangement is not reasonably attainable under circumstances that would not give rise to a conflict of interest, the executive board shall determine by a majority vote of the disinterested directors whether the transaction or arrangement is in the inn between's interest and for our own benefit and whether the transaction is fair and reasonable and shall decide whether to enter into the transaction or arrangement in conformity with such determination. Violations of the conflicts of interest policy. If the board or committee has reasonable cause to believe that a member has failed to disclose actual or possible conflicts of interest, it shall inform the member of the basis for such belief and afford the member an opportunity to explain the alleged failure to disclose. If, after hearing the response of the member and making such further investigation as may be warranted in the circumstances, the board or committee determines that the member has in fact failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective actions. Records of proceedings. The minutes of the board and all committees with board-delegated powers shall contain (1) the names of the persons who disclosed or otherwise were found to have a financial interest or a fiduciary responsibility in connection with an actual or possible conflict of interest, the nature of the financial interest or fiduciary responsibility, any action taken to determine whether a conflict of int

FORM 990 - ORGANIZATION'S MISSION

The inn between's mission is to end the tragic history of vulnerable people dying on the streets of salt lake by providing a safe and supportive haven for people who have nowhere else to go during a medical crisis. The inn between envisions a community where people take care of each other, especially those less fortunate, and where everyone has access to the home-based medical service of professional hospice care at the end of life. The inn between's purpose is to provide a home-like environment where medically frail and terminally ill individuals experiencing homelessness can recuperate from illness, injury or surgery, or experience the end of life with dignity with access to professional hospice care that they cannot receive on the streets. The inn between provides the housing and all medical care is provided by independent state-licensed health care providers. Clients are referred by medical providers and homeless service agencies. All clients must have a documented serious medical condition or terminal diagnosis. The inn between supports the greater community by stabilizing medically frail homeless individuals, which reduces the cost of unnecessary 911 calls and relieves pressure on scarce fire, police and hospital resources. The inn between supports the medical community by shepherding medicaid applications through to completion, which ensures that medical provides are reimbursed for their services.the inn between is not a medical provider and does not receive reimbursement for room and board through our health insurance system, and therefore must raise the funds through philanthropic efforts to sustain its operations. The inn between is a sober-living home that provides clients with all basic needs including food, shelter, clothing, toiletries, an assigned 24/7 bed, transportation to medical appointments, and intensive case management to help clients obtain mainstream benefits such as food stamps, social security disability income, and housing assistance to break the cycle of homelessness. In addition, the program focuses on quality of life by providing comprehensive social and emotional support, death doula services, and activities such as crafts, reiki, acupuncture, music therapy, pet therapy. The program strives to reconnect clients with estranged family members to mend wounds and open the door for forgiveness before it's too late. When a client passes away, the inn between posts an obituary on their website, holds a community memorial service, and places a name plaque in their memorial garden. Having operated at capacity with a long wait list since inception, the inn between sought a larger home. In april 2018, they acquired a former nursing home and relocated their program to the new home on june 1, 2018, ending the lease of their former 16-bed home. The new home was up to code, allowing the inn between to open 25 state licensed assisted living facility type ii beds in addition to 25 eleemosynary (charitable independent living) beds. Assisted living beds are for clients who are so medically frail that they require assistance performing activities of daily living (eating, dressing, bathing, toileting, taking medications), and eleemosynary beds are a continuation of the independent living program that was conducted in the former location. In the new home, the inn between will be able to better meet the community need for hospice and medical respite housing for individuals experiencing homelessness who are terminally ill or too medically frail to be on the streets but not ill enough to be in the hospital. In fy2018, the inn between provided 5,698 medical housing days to 82 unique individuals. The hospice program provided 1,304 housing days to 25 clients, 17 of whom experienced the end of life with dignity with access to professional hospice care, all basic needs, and comprehensive social, emotional and spiritual support, and five of whom carried forward to fy19. The medical respite program provided 4.394 housing days to 57 clients. Seven of our clients were veterans; 38% were women, nine of whom had breast cancer, and six of whom exited the inn between as cancer survivors; and 62% were under the age of 60. IN APRIL 2018, THEY ACQUIRED A FORMER NURSING HOME AND RELOCATED THEIR PROGRAM TO THE NEW HOME ON JUNE 1, 2018, ENDING THE LEASE OF THEIR FORMER 16-BED HOME. THE NEW HOME WAS UP TO CODE, ALLOWING THE INN BETWEEN TO OPEN 25 STATE LICENSED ASSISTED LIVING FACILITY TYPE II BEDS IN ADDITION TO 25 ELEEMOSYNARY (CHARITABLE INDEPENDENT LIVING) BEDS. ASSISTED LIVING BEDS ARE FOR CLIENTS WHO ARE SO MEDICALLY FRAIL THAT THEY REQUIRE ASSISTANCE PERFORMING ACTIVITIES OF DAILY LIVING (EATING, DRESSING, BATHING, TOILETING, TAKING MEDICATIONS), AND ELEEMOSYNARY BEDS ARE A CONTINUATION OF THE INDEPENDENT LIVING PROGRAM THAT WAS CONDUCTED IN THE FORMER LOCATION. IN THE NEW HOME, THE INN BETWEEN WILL BE ABLE TO BETTER MEET THE COMMUNITY NEED FOR HOSPICE AND MEDICAL RESPITE HOUSING FOR INDIVIDUALS EXPERIENCING HOMELESSNESS WHO ARE TERMINALLY ILL OR TOO MEDICALLY FRAIL TO BE ON THE STREETS BUT NOT ILL ENOUGH TO BE IN THE HOSPITAL. IN FY2018, THE INN BETWEEN PROVIDED 5,698 MEDICAL HOUSING DAYS TO 82 UNIQUE INDIVIDUALS. THE HOSPICE PROGRAM PROVIDED 1,304 HOUSING DAYS TO 25 CLIENTS, 17 OF WHOM EXPERIENCED THE END OF LIFE WITH DIGNITY WITH ACCESS TO PROFESSIONAL HOSPICE CARE, ALL BASIC NEEDS, AND COMPREHENSIVE SOCIAL, EMOTIONAL AND SPIRITUAL SUPPORT, AND FIVE OF WHOM CARRIED FORWARD TO FY19. THE MEDICAL RESPITE PROGRAM PROVIDED 4.394 HOUSING DAYS TO 57 CLIENTS. SEVEN OF OUR CLIENTS WERE VETERANS; 38% WERE WOMEN, NINE OF WHOM HAD BREAST CANCER, AND SIX OF WHOM EXITED THE INN BETWEEN AS CANCER SURVIVORS; AND 62% WERE UNDER THE AGE OF 60.
