The inn between is utah's only end of life and medical respite housing solution forterminally ill and medically compromised adults experiencing homelessness. Theprogram opened in august 2015 to address a critical gap in homeless services. The inn between's mission is to end the tragic history of vulnerable people dying on thestreets of our community. They achieve this by providing safe and supportive housing for people who have nowhere to live during a medical crisis. They envision a community where people take care of each other, especially those less fortunate, and where everyone has a safe place in which to 1) experience the end of life with dignity and with access to professional hospice care (a critical medical service that cannot be delivered in shelters, motels or encampments); 2) recuperate from acute medical illness, injury, or surgery; or 3) to gain access to life-saving medical interventions that require stable housing (such as cancer treatment.for terminally ill individuals

The inn between also seeks to reduce costs to our community and relieve pressure on scarce police fire, and hospital resources by reducing unnecessary 911 calls and establishing primary care for our clients. By housing the medically frail and terminally ill, the inn between significantly reduces the misuse of 911 and hospital emergency rooms. The inn between is a 16-bed sober living environment that offers all the comforts of home, such as meals, clothing, bedding, toiletries, laundry and social and emotional support. We are a "surrogate home- and our small staff and many volunteers are like "surrogate family" who provide companionship, transportation to medical appointments, and activities such as music therapy, reiki, acupuncture, grief support, aa/na meetings, jewelry making class, and much more. All medical care is provided by hospice agencies, home health agencies or in clinics, and residents must be referred by a health care professional who can confirm a terminal illness or serious medical condition. The inn between provides terminally ill individuals with a dignified end- of-life experience. We strive to reconnect our residents with estranged family members so they might mend old wounds before it's too late. After a resident dies, we post an obituary on our website, hold a memorial service at our facility and place a name plaque in our memorial garden. The inn between assists medically frail individuals with obtaining mainstream benefits such as social security, medicaid, food stamps and housing assistance to break the cycle of homelessness.”
“In 2016, the inn between's first full year in operation, it provided 4,644 medically appropriate housing days to 75 unique medically frail or terminally ill homeless adults, helping 17 individuals experience the end of life with dignity, safely housed with access to professional medical hospice care. One client was admitted on hospice care, given a few weeks to live. With the assistance and support of the inn between, his body was able to recuperate, he was taken off hospice care, and obtained permanent supportive housing along with a new lease on life. The inn between operated at 80% of capacity, resulting from community outreach and partnerships and demonstrating the community need for the program. 36% of the clients served were admitted on hospice care, 55% received a short term stay to recover from disease or undergo intensive medical treatment, 9% were caregivers (spouses or partners). 4% of the clients served were veterans. The inn between is a 16-bed independent living facility that provides temporary stays to medically frail and terminally ill homeless individuals who require stable housing to recover from disease or experience the end of life with dignity. The mission of the inn between is to end the tragic history of vulnerable people dying on the streets of the salt lake city area community by providing a supportive and safe haven for individuals who have nowhere else to go in time of medical crisis. The inn between provides all basic needs, such as clothing, meals, personal care items and safety, in addition to comprehensive social and emotional support. The inn between ensures that cancer patients have the stable housing required to undergo surgery and intensive treatment such as chemotherapy and radiation. It provides short term stays to individuals who are being discharged from the hospital, but who are too medically frail to return to the streets. And most importantly, it provides compassionate and supportive housing to ensure that individuals diagnosed with a terminal illness can receive hospice care (a home-based service) and experience the end of life in safety and with dignity. The inn between makes every effort to reunite individuals with family members who may be estranged, so they can make amends before it's too late. After a resident passes away, the inn between posts an obituary on their website, holds a community memorial service and places a name plaque in their memorial garden, all at no cost to the client or his/her family. When cremains go unclaimed, the inn between collects them in the hope of reuniting them with a family member who may find the obituary at a later date.”

ALL MEDICAL CARE IS PROVIDED BY HOSPICE AGENCIES, HOME HEALTH AGENCIES OR IN CLINICS, AND RESIDENTS MUST BE REFERRED BY A HEALTH CARE PROFESSIONAL WHO CAN CONFIRM A TERMINAL ILLNESS OR SERIOUS MEDICAL CONDITION. THE INN BETWEEN PROVIDES TERMINALLY ILL INDIVIDUALS WITH A DIGNIFIED END- OF-LIFE EXPERIENCE. WE STRIVE TO RECONNECT OUR RESIDENTS WITH ESTRANGED FAMILY MEMBERS SO THEY MIGHT MEND OLD WOUNDS BEFORE IT'S TOO LATE. ONE CLIENT WAS ADMITTED ON HOSPICE CARE, GIVEN A FEW WEEKS TO LIVE. WITH THE ASSISTANCE AND SUPPORT OF THE INN BETWEEN, HIS BODY WAS ABLE TO RECUPERATE, HE WAS TAKEN OFF HOSPICE CARE, AND OBTAINED PERMANENT SUPPORTIVE HOUSING ALONG WITH A NEW LEASE ON LIFE. THE INN BETWEEN OPERATED AT 80% OF CAPACITY, RESULTING FROM COMMUNITY OUTREACH AND PARTNERSHIPS AND DEMONSTRATING THE COMMUNITY NEED FOR THE PROGRAM. 36% OF THE CLIENTS SERVED WERE ADMITTED ON HOSPICE CARE, 55% RECEIVED A SHORT TERM STAY TO RECOVER FROM DISEASE OR UNDERGO INTENSIVE MEDICAL TREATMENT, 9% WERE CAREGIVERS (SPOUSES OR PARTNERS). 4% OF THE CLIENTS SERVED WERE VETERANS. THE INN BETWEEN IS A 16-BED INDEPENDENT LIVING FACILITY THAT PROVIDES TEMPORARY STAYS TO MEDICALLY FRAIL AND TERMINALLY ILL HOMELESS INDIVIDUALS WHO REQUIRE STABLE HOUSING TO RECOVER FROM DISEASE OR EXPERIENCE THE END OF LIFE WITH DIGNITY. THE MISSION OF THE INN BETWEEN IS TO END THE TRAGIC HISTORY OF VULNERABLE PEOPLE DYING ON THE STREETS OF THE SALT LAKE CITY AREA COMMUNITY BY PROVIDING A SUPPORTIVE AND SAFE HAVEN FOR INDIVIDUALS WHO HAVE NOWHERE ELSE TO GO IN TIME OF MEDICAL CRISIS. THE INN BETWEEN PROVIDES ALL BASIC NEEDS, SUCH AS CLOTHING, MEALS, PERSONAL CARE ITEMS AND SAFETY, IN ADDITION TO COMPREHENSIVE SOCIAL AND EMOTIONAL SUPPORT. THE INN BETWEEN ENSURES THAT CANCER PATIENTS HAVE THE STABLE HOUSING REQUIRED TO UNDERGO SURGERY AND INTENSIVE TREATMENT SUCH AS CHEMOTHERAPY AND RADIATION. IT PROVIDES SHORT TERM STAYS TO INDIVIDUALS WHO ARE BEING DISCHARGED FROM THE HOSPITAL, BUT WHO ARE TOO MEDICALLY FRAIL TO RETURN TO THE STREETS. AND MOST IMPORTANTLY, IT PROVIDES COMPASSIONATE AND SUPPORTIVE HOUSING TO ENSURE THAT INDIVIDUALS DIAGNOSED WITH A TERMINAL ILLNESS CAN RECEIVE HOSPICE CARE (A HOME-BASED SERVICE) AND EXPERIENCE THE END OF LIFE IN SAFETY AND WITH DIGNITY. THE INN BETWEEN MAKES EVERY EFFORT TO REUNITE INDIVIDUALS WITH FAMILY MEMBERS WHO MAY BE ESTRANGED, SO THEY CAN MAKE AMENDS BEFORE IT'S TOO LATE. AFTER A RESIDENT PASSES AWAY, THE INN BETWEEN POSTS AN OBITUARY ON THEIR WEBSITE, HOLDS A COMMUNITY MEMORIAL SERVICE AND PLACES A NAME PLAQUE IN THEIR MEMORIAL GARDEN, ALL AT NO COST TO THE CLIENT OR HIS/HER FAMILY.
