Mission and Program Overview

Mission

The inn between is utah's only end of life and medical respite housing solution forterminally ill and medically compromised adults experiencing homelessness. Theprogram opened in august 2015 to address a critical gap in homeless services. The inn between's mission is to end the tragic history of vulnerable people dying on thestreets of our community. They achieve this by providing safe and supportive housing for people who have nowhere to live during a medical crisis. They envision a community where people take care of each other, especially those less fortunate, and where everyone has a safe place in which to 1) experience the end of life with dignity and with access to professional hospice care (a critical medical service that cannot be delivered in shelters, motels or encampments); 2) recuperate from acute medical illness, injury, or surgery; or 3) to gain access to life-saving medical interventions that require stable housing (such as cancer treatment.for terminally ill individuals

FORM 990 - ORGANIZATION'S MISSION

The inn between, utah's first hospice for the homeless, opened on august 17, 2015, bridging a longstanding gap in end-of-life housing for homeless individuals along the wasatch front. The inn between's mission is to offer a safe and comfortable place were utah's terminally ill homeless men and women can experience the end-of-life with dignity, receive professional hospice services and be surrounded by people who care. We envision a community where everyone has access to the in-home service of hospice care and where no one dies alone on the street. The inn between's purpose is to provide a home-like environment where medically frail and terminally ill homeless adults, and their homeless spouses or caregivers and even their pets, can recuperate from illness or experience death with dignity. The inn between gives homeless individuals access to health care and hospice care that they could not receive on the streets. All medical care is provided by independent state-licensed agencies. The inn between also seeks to reduce costs to our community and relieve pressure on scarce police fire, and hospital resources by reducing unnecessary 911 calls and establishing primary care for our clients. By housing the medically frail and terminally ill, the inn between significantly reduces the misuse of 911 and hospital emergency rooms. The inn between is a 16-bed sober living environment that offers all the comforts of home, such as meals, clothing, bedding, toiletries, laundry and social and emotional support. We are a "surrogate home- and our small staff and many volunteers are like "surrogate family" who provide companionship, transportation to medical appointments, and activities such as music therapy, reiki, acupuncture, grief support, aa/na meetings, jewelry making class, and much more. All medical care is provided by hospice agencies, home health agencies or in clinics, and residents must be referred by a health care professional who can confirm a terminal illness or serious medical condition. The inn between provides terminally ill individuals with a dignified end- of-life experience. We strive to reconnect our residents with estranged family members so they might mend old wounds before it's too late. After a resident dies, we post an obituary on our website, hold a memorial service at our facility and place a name plaque in our memorial garden. The inn between assists medically frail individuals with obtaining mainstream benefits such as social security, medicaid, food stamps and housing assistance to break the cycle of homelessness.

Form 990, Page 2, Part III, Line 4A

The year 2015 marked the launch of the inn between's 16-bed home. The organization was incorporated and granted irs 501(c) (3) nonprofit status as of december 3, 2014. The facility opened on august 17, 2015, and they admitted their first hospice resident, and her spouse, that day. Between august 17, 2015 and december 31, 2015, the inn between helped 3 people, one woman age 31 and two men ages 32 and 58, experience death with dignity in a home-like environment. By living at the inn between, these individuals were able to receive in-home hospice care to ease the pain, suffering and anxiety at the end-of-life. In the same period, the inn between provided a total of 1,290 medically appropriate housing nights to 22 unduplicated terminally ill or medically frail individuals along with 5 caregivers, for a total of 27 people served. Two clients left then returned, for a total of 29 service incidents. The inn between posted obituaries for the deceased on its website and laced name plaques in their memorial garden. The inn between also held memorial services for two of the deceased, with the family of the third opting out. The inn between estimates that it prevented at least one unnecessary 911 call and emergency room visit per resident, which saved the community 44,000. To maximize our impact on homelessness, the inn between collaborates with governmental agencies and other nonprofit organizations which fall under the 501(c)(3) section of the irs code and are operated exclusively for educational and charitable purposes. The inn between provides volunteer, service learning, internship and community service opportunities to maximize our program outcomes and have a greater positive impact on the community.
