“Enforcement of conflicts policy disclosure as to any conflict of interest(or confirmation that there are not any conflicts of interest) must be made annually in writing by all officers and directors, and responsible individuals. In the event that a conflict of interest arises that has not been previously disclosed, the responsible individual shall disclose such conflict of interest as soon as possible. All information disclosed shall remain on file in the corporate records of the organization.”
