Making choices michigan will remain as a seperate legal entity, but will be a wholly owned subsidiary of great lakes health connect. Effective january 1, 2018 making choices michigan will be included as part of the form 990 for great lakes health connect.”

“To create a culture that accepts every person's right to make choices about the health care they receive throughout life, including at the end of life, and feel assured that those choices will be respected. We are committed to encouraging and facilitating advance care planning in michigan and surrounding communities. Advance care planning is a process of engaging individuals and their families or friends in a discussion on the individual's preferences for their health care.”

“Making choices michigan's (mcm) mission is to encourage and facilitate advance health care planning (acp) by the people of michigan, with the goal of having individuals discuss the care they would chose if they are unable to speak for themselves. In 2017, mcm continued to expand its community outreach efforts through the interfaith acp program. In addition to mcm's usual outreach endeavors, the mcm-trained first step facilitators presented acp education to their congregants, followed by facilitated acp conversation with willing participants. By 2017 year-end, mcm volunteer facilitators and the interfaith facilitators had educated 1,320+ individuals, received 235 conversation requests, facilitated 155 conversations, and completed 69 advance directives (ad). In addition, mcm referred 8 conversations to local spanish-speaking or outside healthcare provider facilitators. Finally, mcm trained 34 new first-step facilitators in 2017. Of note, the majority are still working part or full time in their chosen profession, and see mcm as an important community initiative worthy of their volunteer hours.”
