“To create a culture that accepts every person's right to make choices about the health care they receive throughout life, including at the end of life, and feel assured that those choices will be respected. We are committed to encouraging and facilitating advance care planning in kent county, michigan and surrounding communities. Advance care planning is a process of engaging individuals and their families or friends in a discussion on the individual's preferences for their health care.”

“Making choices michigan's (mcm) mission is to encourage and facilitate advance health care planning (acp) by the people of west michigan. In 2015, mcm developed tools and resources to provide acp education, including public presentation tools for the mcm speakers bureau, acp brochure and the mcm website. In addition, the mcm common advance directive document was refined based on 2014 community engagement pilot feedback. Through partnerships with various professional and public groups, mcm delivered over 63 acp education events to over 1,874 individuals. Mcm received 279 requests for a facilitated acp conversation. By year-end, over 200 of those conversations were completed, with the remainder "in process." our acp participants ranged in age from 20 to 91+, with the majority between the ages of 61-80. Females comprised 69% of the participant sample. Mcm volunteers completed one third of the conversations, with mcm staff facilitating the remainder of conversations. Spanish only facilitation requests are on the rise, and have been a challenge to fulfill. At this point, only 2 of the 12 volunteers are bilingual.”
