To create a culture that accepts every person's right to make choices about the health care they receive throughout life, including at the end of life, and feel assured that those choices will be respected. We are committed to encouraging and facilitating advance care planning in kent county, michigan and surrounding communities. Advance care planning is a process of engaging individuals and their families or friends in a discussion on the individual's preferences for their health care.

MAKING CHOICES MICHIGAN IS WORKING COLLABORATIVELY WITH AREA HOSPITALS, SCHOOLS OF NURSING, MEDICAL SCHOOLS, HOSPICE ORGANIZATIONS, SENIOR LIVING COMMUNITIES AND THE LEGAL COMMUNITY TO BUILD A COMMUNITY INFRASTRUCTURE TO ADVANCE OUR MISSION OF HONORING A PERSON'S WISHES FOR HEALTH CARE AT THE END OF LIFE. DURING THE LAST YEAR, WE TRAINED AN ADDITIONAL 37 FACILITATORS WHICH GIVES US A TOTAL OF 92 FACILITATORS THAT HAVE BEEN TRAINED TO ASSIST PERSONS IN DISCUSSIONS ABOUT THEIR END-OF-LIFE CHOICES FOR HEALTH CARE. THEY ARE CURRENTLY AT WORK IN SENIOR LIVING FACILITIES, PRIMARY CARE OFFICES, CANCER CLINICS AND IN THE COMMUNITY. A COMMUNITY EDUCATION AND ENGAGEMENT PLAN HAS BEEN DEVELOPED THAT WILL INFORM OUR COMMUNITY OF THE IMPORTANCE OF HAVING THESE CONVERSATIONS WITH THEIR LOVED ONES. A NUMBER OF PUBLIC EVENTS HAVE TAKEN PLACE TO HIGHLIGHT THIS WORK TO THE COMMUNITY. Advance care planning is a process of engaging individuals and their families or friends in a discussion on the individual's preferences for their health care.

Form 990-ez, Part III, Line 28

Making choices michigan is working collaboratively with area hospitals, schools of nursing, medical schools, hospice organizations, senior living communities and the legal community to build a community infrastructure to advance our mission of honoring a person's wishes for health care at the end of life. During the last year, we trained an additional 37 facilitators which gives us a total of 92 facilitators that have been trained to assist persons in discussions about their end-of-life choices for health care. They are currently at work in senior living facilities, primary care offices, cancer clinics and in the community. A community education and engagement plan has been developed that will inform our community of the importance of having these conversations with their loved ones. A number of public events have taken place to highlight this work to the community. An electronic storage and retrieval system has been developed that will assist medical providers in obtaining a patient's advance directive during a medical emergency. Common documents have been developed which will be used throughout the community and will enhance the development of a unified, community-wide system for advance care planning. In addition, making choices michigan continues to work collaboratively with other organizations in michigan to enhance this initiative throughout the state.

WE ARE COMMITTED TO ENCOURAGING AND FACILITATING ADVANCE CARE PLANNING IN KENT COUNTY, MICHIGAN AND SURROUNDING COMMUNITIES. ADVANCE CARE PLANNING IS A PROCESS OF ENGAGING INDIVIDUALS AND THEIR FAMILIES OR FRIENDS IN A DISCUSSION ON THE INDIVIDUAL'S PREFERENCES FOR THEIR HEALTH CARE.
IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt4MAKING CHOICES MICHIGAN IS WORKING COLLABORATIVELY WITH AREA HOSPITALS, SCHOOLS OF NURSING, MEDICAL SCHOOLS, HOSPICE ORGANIZATIONS, SENIOR LIVING COMMUNITIES AND THE LEGAL COMMUNITY TO BUILD A COMMUNITY INFRASTRUCTURE TO ADVANCE OUR MISSION OF HONORING A PERSON'S WISHES FOR HEALTH CARE AT THE END OF LIFE. DURING THE LAST YEAR, WE TRAINED AN ADDITIONAL 37 FACILITATORS WHICH GIVES US A TOTAL OF 92 FACILITATORS THAT HAVE BEEN TRAINED TO ASSIST PERSONS IN DISCUSSIONS ABOUT THEIR END-OF-LIFE CHOICES FOR HEALTH CARE. THEY ARE CURRENTLY AT WORK IN SENIOR LIVING FACILITIES, PRIMARY CARE OFFICES, CANCER CLINICS AND IN THE COMMUNITY. A COMMUNITY EDUCATION AND ENGAGEMENT PLAN HAS BEEN DEVELOPED THAT WILL INFORM OUR COMMUNITY OF THE IMPORTANCE OF HAVING THESE CONVERSATIONS WITH THEIR LOVED ONES. A NUMBER OF PUBLIC EVENTS HAVE TAKEN PLACE TO HIGHLIGHT THIS WORK TO THE COMMUNITY. AN ELECTRONIC STORAGE AND RETRIEVAL SYSTEM HAS BEEN DEVELOPED THAT WILL ASSIST MEDICAL PROVIDERS IN OBTAINING A PATIENT'S ADVANCE DIRECTIVE DURING A MEDICAL EMERGENCY. COMMON DOCUMENTS HAVE BEEN DEVELOPED WHICH WILL BE USED THROUGHOUT THE COMMUNITY AND WILL ENHANCE THE DEVELOPMENT OF A UNIFIED, COMMUNITY-WIDE SYSTEM FOR ADVANCE CARE PLANNING. IN ADDITION, MAKING CHOICES MICHIGAN CONTINUES TO WORK COLLABORATIVELY WITH OTHER ORGANIZATIONS IN MICHIGAN TO ENHANCE THIS INITIATIVE THROUGHOUT THE STATE.
