Form 990, Part III, Line 2

Lyngblomsten community services launched a pilot program for providing in-home respite services for older adults living with memory loss and their caregivers. As part of the program, volunteers are trained to spend time in the homes of older adults as respite companions. The pilot was funded through a live well at home grant from the minnesota department of human services. To support aging in place, home health services expanded service to tenants of the heritage at lyngblomsten apartments. Accompanied by a wellness fee, enhanced services include weekly wellness checks, delivery of meals and escorts to meals for 48 hours following a significant condition change or hospital discharge, and a light cleaning of one's entire apartment once a month (bathroom is cleaned twice per month). See narratives on individual program areas below for additional highlights.

Lyngblomsten community services received a grant from the lutheran services for the elderly endowment through the elca foundation to develop and launch an online training portal for congregational volunteers who serve older adults in the community through care teams. These teams assist older adults with errands, transportation, housekeeping, and other needs. Called the care ministry network, this online portal is expected to launch in 2019. 2nd half with lyngblomsten (a service model combining all of our previously separate community-based programs) completed its second full year of operations in 2018. During this time, several new evidence-based health and wellness programs debuted.

PART X, LINE 2:

The organization has been granted exempt status relative to federal and minnesota corporate income taxes under section 501(c)(3) of the internal revenue code and applicable state codes. The organization follows the guidance in the income tax standard regarding the recognition and measurement of uncertain tax positions. This prescribes recognition and measurement of tax provisions taken or expected to be taken on a tax return that are not certain to be realized. The organization is not aware of any activities that would jeopardize its tax-exempt status. The organization reports any activities that are subject to tax on unrelated business income or excise or other taxes and files all proper returns related to these activities.
