Form 990, Part III, Line 2

Advancing family assets (afa)- uses a family success coaching model to help local families reach goals in the areas of income, education, and health. During their 9-12 month involvement in afa, each family works with a coach to focus on their best possible futures, helping: 1. Adults to get jobs or better jobs 2. Families to manage their money better 3. Young children to be ready to succeed in kindergarten 4. Kids to do better in school and participate in afterschool activities 5. Family members to learn to get along with each other 6. All family members to practice healthy lifestyles. Ultimately, adults will be prepared to succeed in the workplace and students will graduate ready to enter the workforce or post-secondary education without remediation. Afa helps give families chances for success. When families succeed, our community succeeds. Multihub fast - this is a novel type of fast (families and schools together) program, which is an innovative and collaborative prevention and parent-involvement program in which families gather to participate in specific, fun, research-based activities aimed at strengthening families and empowering parents. Schools, families, and two community agencies join together in eight weekly sessions. The sessions held at local schools, are about 2 hours long, and include a meal. The "multi-hub" in the name refers to the fact that more than one fast session is delivered simultaneously within a school. Safe schools healthy students fast - very similar to the other fast programs (united way and multihub), except that it is a smaller program and the primary funding is from a different source (our local school district). The fast sessions are the same as the multi-hub fast, without the multi-hub (more than one session at the same time) approach. Opiate prevention - is aimed at educating our community regarding opiate abuse, garnering support from all community sectors and developing systems in an effort to prevent this growing epidemic. Strengthening families - also known as "strengthening families program for 10-14". There are seven weeks of meetings where parents learn to show love and set limits for their children, while helping their youth develop skills in handling peer pressure and building a positive future. Parents and youth attend separate skill-building groups for the first hour and spend the second hour together in supervised family activities. Smart recovery - also known as "smart recovery for youth". This is a self-empowering additional recovery support group. Participants learn tools for association recovery based on the latest scientific research and participate in a world-wide community which includes self-empowering science-based mutual help groups. Rcyc - requested a mini-grant to support teen institute, a four day, three night leadership retreat geared toward helping the young leaders of our community meet other youth with a passion for being involved. The retreat consists of several workshops that cover issues like diversity, youth in government, substance abuse prevention, and personal development skills.

PART X, LINE 2:

Focus on community, inc is exempt from federal income taxes under section 501(c)(3) of the internal revenue code, and has been established as a non-profit organization under chapter 181 of the wisconsin statutes. The organization adopted accounting for uncertainty in income taxes guidance fasb asc 740-10. The adoption of that guidance resulted in no change to net assets or any related tax positions. Management continually evaluates expiring statues of limitations, audits, changes in tax law, and new authoritative rulings. The organization has no unrelated business income tax, accrued interest or penalties associated with uncertain tax positions. None of the organization's federal or state informational returns are currently under examination.

DURING THEIR 9-12 MONTH INVOLVEMENT IN AFA, EACH FAMILY WORKS WITH A COACH TO FOCUS ON THEIR BEST POSSIBLE FUTURES, HELPING: 1. ADULTS TO GET JOBS OR BETTER JOBS 2. FAMILIES TO MANAGE THEIR MONEY BETTER 3. YOUNG CHILDREN TO BE READY TO SUCCEED IN KINDERGARTEN 4. KIDS TO DO BETTER IN SCHOOL AND PARTICIPATE IN AFTERSCHOOL ACTIVITIES 5. THE ORGANIZATION HAS NO UNRELATED BUSINESS INCOME TAX, ACCRUED INTEREST OR PENALTIES ASSOCIATED WITH UNCERTAIN TAX POSITIONS. NONE OF THE ORGANIZATION'S FEDERAL OR STATE INFORMATIONAL RETURNS ARE CURRENTLY UNDER EXAMINATION.
