Mission

Our vision is to be a leader in behavioral healthcare by providing recovery-focused evidence-based integrated mental health and primary care services for more than 900 clients. We have an appreciation for the high prevalence of traumatic experiences in persons who receive mental health services. Every person served by our organization experiences a safe and trusting physical and social environment that provides trauma-informed and trauma-specific services that are easily accessible; person-centered, competently delivered, culturally respectful and consistent with recovery-oriented principles of collaboration: shared decision making and self-direction. We offer a wide-array of services, including crisis mental health services, person-centered treatment planning, integrated healthcare, psychiatric services, integrated dual disorders treatment, psychosocial rehabilitation services, residential services, housing coordination, supported employment, peer support, and family psychoeducation.

Form 990, Part VI, Section B, Line 11

It is the policy of trilogy, inc. That the finance committee will be responsible for the initial review of the internal revenue service form 990 that is to be filed on behalf of the organization. The completed form 990 will be provided to the finance committee of the board of directors sufficiently in advance of the filing deadline to enable a detailed and conscientious review by all members of the committee. All questions, concerns, etc. Of the finance committee members will be addressed by the chief financial officer and incorporated into the form 990 as appropriate. Upon the approval of the finance committee, the form 990 shall be presented to the board of directors noting a recommendation from the finance committee to the executive committee for approval of the form 990 for filing with the irs. After all of the input from the board of directors and the finance committee has been appropriately addressed, senior management will file the final form 990 as required.

All officers, directors, and key employees are required to sign a conflict of interest disclosure and if any parties become aware of any actual or potential conflict of interest, must notify the president/ceo in writing.

Form 990, Part VI, Section B, Line 15

The purpose of the performance review of the president & ceo is to: 1. Ensure consistency and integrity of the process; 2. Attract and retain a new president & ceo when necessary; 3. Attend to succession planning; 4. Ensure the stewardship of the agency by protecting it (both board and president & ceo). Policy: it is the policy of the board to review the performance of the president & ceo annually. The review will ensure that these procedures are followed each time a review is undertaken. The board members jointly responsible for ensuring this takes place are the board president and the chair of the human resources (hr) committee, who may be assisted by members of the executive committee as requested. Time frame: the president & ceo shall initiate the performance review process for the prior year completed on june 30 after july 1 but no later than october 31 each year. It must be completed by december 1 each year. Procedures: 1. The president & ceo will provide a written review of the goals and objectives for the prior fiscal year per the strategic plan and the steps taken to reach them. Explanations should be provided in the event certain goals have not been met. The review process must include objective criteria and measurements. Special achievements, which were not part of the original goals for the year, must be included in the review. Soft or subjective issues should also be reviewed as part of the performance review template. 2. The board chairperson will review the written document prepared by the president & ceo and request any clarifications that might be needed. 3. The board chairperson will then inform board members of the plan to evaluate the president & ceo and will invite them to submit feedback by a set date; 4. President & ceo and board chairperson shall meet face to face for a conversation about the president & ceo's performance. This conversation shall include the feedback from board members. 5. The board chairperson will produce a written summary of the meeting; 6. Board chairperson will consult with executive committee chair to include his/her inputs. 7. Board chairperson then takes the recommendation from above to the executive committee (including finance chair) in executive session in order to draft the final recommendation. The recommendation will include the change in the president & ceo's compensation package proposed by the board chairperson and hr chair and put in its final form by the executive committee in executive session. 8. The president & ceo may prepare a written response to provide additional information or to counter the review content and present this to the members of the executive committee; 9. A list of formal goals should be developed from the review for the coming year in addition to the annual strategic goals) and a date set for a mid year review; 10. Goals should support the personal/professional development of the president & ceo, at the discretion of the board. This policy/procedure is set by the board and cannot be changed without approval by the board. The salary/benefits package is reviewed annually as a function of this process. The procedure is to: 1. Review trends and data from surveys (as current as possible) and make adjustments based on market changes and the unique situations; 2. The board chairperson in consultation with individuals with qualified hr experience prepares a review and presents the review and compensation adjustments to the president & ceo, after review of the summary and concurrence by the executive committee; 3. The president & ceo and the board chairperson sign the applicable forms to enact the changes (note that effective dates of salary change are typically to fiscal year).

PART X, LINE 2:

The organization follows authoritative guidance issued by fasb that clarifies the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a recognition threshold of more-likely-than-not to be sustained upon examination. Measurement of the tax uncertainty occurs if the recognition threshold has not been met. This guidance also addresses derecognition, classification, interest and penalties, disclosure, and transition. The organization conducts business solely in the u.s. And, as a result, files information returns for u.s. And illinois. In the normal course of business, the organization is subject to examination by taxing authorities. The organization's information returns for years subsequent to fiscal 2010 are open, by statute, for review by authorities. However, at present, there are no ongoing income tax audits or unresolved disputes with the various tax authorities that the organization currently files or has filed with.

AND, AS A WILLIAMS CLASS MEMBER PROVIDER- ASSISTING MEN AND WOMEN WHO HAVE LIVED IN A NURSING HOME SETTING FOR LARGE PORTIONS OF THEIR LIVES WITH THE TRANSITION OF MOVING INTO THEIR OWN APARTMENTS AS PART OF THE IMPLEMENTATION PLAN SET FORTH BY THE STATE OF ILLINOIS' WILLIAMS CONSENT DECREE. TRILOGY HAS TRANSITIONED MORE THAN 110 INDIVIDUALS FROM NURSING HOMES INTO INDEPENDENT HOUSING.EMPLOYMENT SERVICES - IN 2014, TRILOGY RECEIVED THE HIGHEST FIDELITY SCORE IN THE HISTORY OF THE STATE OF ILLINOIS FOR OUR SUPPORTED EMPLOYMENT PROGRAM. TRILOGY IS A LEARNING PARTNER IN THE JOHNSON & JOHNSON DARTMOUTH COMMUNITY MENTAL HEALTH PROGRAM, ALSO CALLED THE INDIVIDUAL PLACEMENT AND SUPPORT (IPS) SUPPORTED EMPLOYMENT LEARNING COLLABORATIVE. THE IPS MODEL FOR SUPPORTED EMPLOYMENT IS THE MOST PROGRESSIVE, EVIDENCE-BASED MODEL TO BE OFFERED TO INDIVIDUALS WITH SERIOUS AND PERSISTENT MENTAL ILLNESS AND IS AVAILABLE TO MORE THAN 900 TRILOGY CLIENTS. IMPORTANTLY, STAFF AND RECIPIENTS FIND INDIVIDUALIZED JOB PLACEMENTS BASED ON RECIPIENT PREFERENCES, STRENGTHS, AND WORK EXPERIENCES. AN IMPORTANT FEATURE OF SUPPORTED EMPLOYMENT IS EQUITABLE COMPENSATION: WAGES AND BENEFITS MUST BE THE SAME AS OTHER WORKERS IN SIMILAR JOBS RECEIVE. OUR EMPLOYMENT SPECIALISTS (ES) IDENTIFY AND BUILD RELATIONSHIPS WITH COMMUNITY-BASED EMPLOYERS AND WORKSITES TO DEVELOP JOB PLACEMENTS AND SETTINGS IDENTIFIED BY THE CLIENTS. THE ES MAY SERVE AS A JOB COACH, SUPPORTING CLIENTS THROUGH THE PROCESS OF SEARCHING FOR AND ADJUSTING TO EMPLOYMENT. THE PROGRAM ASSISTS A WIDE-RANGE OF PEOPLE, FROM THOSE WHO HAVE NEVER WORKED AND MAY NEED THE ENTIRE CONTINUUM OF TRAINING AND SUPPORT, TO THOSE WHO HAVE EXTENSIVE PRIOR WORK HISTORIES AND REQUIRE MINIMUM SUPPORT TO RETURN TO THE WORKFORCE.PEER SERVICES - THE TRILOGY BEACON IS A SEVEN DAY A WEEK 365 DAY PER YEAR COMMUNITY DROP-IN CENTER THAT OFFERS PEER-TO-PEER LEADERSHIP AND IS OPEN TO ANY PERSON PRESENTING AS AN INDIVIDUAL WITH SERIOUS MENTAL ILLNESS. IN 2010, TRILOGY WAS AWARDED A $330,000 THREE-YEAR COMPETITIVE TITLE XX GRANT THAT ALLOWED US TO SIGNIFICANTLY ENHANCE OUR DROP-IN CENTER BY HIRING THREE FULL-TIME AND TWO PART-TIME PEER SPECIALISTS - MENTAL HEALTH CONSUMERS WHO HAVE COMPLETED SPECIFIC TRAINING THAT ENABLES THEM TO ENHANCE A PERSON'S WELLNESS AND RECOVERY BY PROVIDING PEER SUPPORT - AND AN EMPLOYMENT SPECIALIST - A RECOVERY COUNSELOR WHO ASSISTS WITH JOB EXPLORATION, JOB SEARCHING, RESUME WRITING, MOCK INTERVIEWS, AND JOB COACHING. THE INITIAL GOAL OF THE BEACON WAS TO SERVE MORE THAN 400 UNDUPLICATED PARTICIPANTS PER YEAR. ONE OF THE GREAT SUCCESSES OF THE TRILOGY BEACON IS THAT WE CURRENTLY SERVE MORE THAN 1500 GUESTS PER MONTH - A LARGE NUMBER BEING "NON-REGISTERED" CLIENTS FROM THE ROGERS PARK COMMUNITY WHO ARE HOMELESS OR WITHOUT MEDICAID BENEFITS.
