Mission and Program Overview

Mission

Our vision is to be a leader in behavioral healthcare by providing recovery-focused evidence-based integrated mental health and primary care services for more than 900 clients. We have an appreciation for the high prevalence of traumatic experiences in persons who receive mental health services. Every person served by our organization experiences a safe and trusting physical and social environment that provides trauma-informed and trauma-specific services that are easily accessible; person-centered, competently delivered, culturally respectful and consistent with recovery-oriented principles of collaboration: shared decision making and self-direction. We offer a wide-array of services, including crisis mental health services, person-centered treatment planning, integrated healthcare, psychiatric services, integrated dual disorders treatment, psychosocial rehabilitation services, residential services, housing coordination, supported employment, peer support, and family psychoeducation.

Major Activities

Activity 2
Provide psychosocial rehabilitation for about 480 adults with psychiatric disabilities. Due to budget cuts, trilogy eliminated four managerial positions. The duties of the managers were shifted to team leaders (as the first level supervisors) and to the executive-level staff. Another consequence of the restructuring process was the elimination of our psychological training program, which required supervision by a licensed clinical psychologist. All psychology positions were eliminated.
Activity 3
Linkage outreach team provides services for about 480 adults with psychiatric disabilities. In fy09, trilogy's client population began to shift towards clients with higher needs. In order to be able to meet the needs of these clients, an additional linkage/outreach team was added. This team focuses on serving high-need clients including homeless clients, clients with legal issues, and clients who are not linked to entitlements. These services are provided primarily in the community.

THE IPS MODEL FOR SUPPORTED EMPLOYMENT IS THE MOST PROGRESSIVE, EVIDENCE-BASED MODEL TO BE OFFERED TO INDIVIDUALS WITH SERIOUS AND PERSISTENT MENTAL ILLNESS AND IS AVAILABLE TO MORE THAN 900 TRILOGY CLIENTS. IMPORTANTLY, STAFF AND RECIPIENTS FIND INDIVIDUALIZED JOB PLACEMENTS BASED ON RECIPIENT PREFERENCES, STRENGTHS, AND WORK EXPERIENCES. OUR EMPLOYMENT SPECIALISTS (ES) IDENTIFY AND BUILD RELATIONSHIPS WITH COMMUNITY-BASED EMPLOYERS AND WORKSITES TO DEVELOP JOB PLACEMENTS AND SETTINGS IDENTIFIED BY THE CLIENTS. THE ES MAY SERVE AS A JOB COACH, SUPPORTING CLIENTS THROUGH THE PROCESS OF SEARCHING FOR AND ADJUSTING TO EMPLOYMENT. THE PROGRAM ASSISTS A WIDE-RANGE OF PEOPLE, FROM THOSE WHO HAVE NEVER WORKED AND MAY NEED THE ENTIRE CONTINUUM OF TRAINING AND SUPPORT, TO THOSE WHO HAVE EXTENSIVE PRIOR WORK HISTORIES AND REQUIRE MINIMUM SUPPORT TO RETURN TO THE WORKFORCE.PEER SERVICES - THE TRILOGY BEACON IS A SEVEN DAY A WEEK 365 DAY PER YEAR COMMUNITY DROP-IN CENTER THAT OFFERS PEER-TO-PEER LEADERSHIP AND IS OPEN TO ANY PERSON PRESENTING AS AN INDIVIDUAL WITH SERIOUS MENTAL ILLNESS. ONE OF THE GREAT SUCCESSES OF THE TRILOGY BEACON IS THAT WE CURRENTLY SERVE MORE THAN 1500 GUESTS PER MONTH - A LARGE NUMBER BEING "NON-REGISTERED" CLIENTS FROM THE ROGERS PARK COMMUNITY WHO ARE HOMELESS OR WITHOUT MEDICAID BENEFITS. AS PART OF A CITY OF CHICAGO'S DEPARTMENT OF PUBLIC HEALTH'S (CDPH'S) MINORITY AIDS INITIATIVE TARGET CAPACITY EXPANSION (MAI-TCE) INTEGRATED BEHAVIORAL HEALTH/PRIMARY CARE GRANT AWARD, TRILOGY IS PROUD TO PARTNER WITH CHICAGO HOUSE-AN AGENCY WHICH "PROVIDES HOUSING AND SUPPORTIVE SERVICES TO HIV-AFFECTED AND HIV-AT RISK FAMILIES AND INDIVIDUALS, WHO MAY ALSO BE IMPACTED BY POVERTY, HOMELESSNESS, SUBSTANCE ABUSE, AND MENTAL ILLNESS"- TO PROVIDE AIDS TESTING AND COUNSELING WITHIN THE TRILOGY BEACON.
