Mission

Our vision is to be a leader in behavioral healthcare by providing recovery-focused evidence-based integrated mental health and primary care services for more than 900 clients. We have an appreciation for the high prevalence of traumatic experiences in persons who receive mental health services. Every person served by our organization experiences a safe and trusting physical and social environment that provides trauma-informed and trauma-specific services that are easily accessible; person-centered, competently delivered, culturally respectful and consistent with recovery-oriented principles of collaboration: shared decision making and self-direction. We offer a wide-array of services, including crisis mental health services, person-centered treatment planning, integrated healthcare, psychiatric services, integrated dual disorders treatment, psychosocial rehabilitation services, residential services, housing coordination, supported employment, peer support, and family psychoeducation.

Governance Explanations

Form 990, Part VI, Section B, Line 11

It is the policy of trilogy, inc. That the finance committee will be responsible for the initial review of the internal revenue service form 990 that is to be filed on behalf of the organization. The completed form 990 will be provided to the finance committee of the board of directors sufficiently in advance of the filing deadline to enable a detailed and conscientious review by all members of the committee. All questions, concerns, etc. Of the finance committee members will be addressed by the chief financial officer and incorporated into the form 990 as appropriate. Upon the approval of the finance committee, the form 990 shall be presented to the board of directors noting a recommendation from the finance committee to the executive committee for approval of the form 990 for filing with the irs. After all of the input from the board of directors and the finance committee has been appropriately addressed, senior management will file the final form 990 as required.

Form 990, Part VI, Section B, Line 12C

All officers, directors, and key employees are required to sign a conflict of interest disclosure and if any parties become aware of any actual or potential conflict of interest, must notify the president/ceo in writing.

Form 990, Part VI, Section B, Line 15A

The president and ceo's annual performance evaluation will begin in mid-september of each year following these timelines: 1. Vp/hr provides executive committee with reports on ceo's 360 reviews completed by board members and senior management, client satisfaction surveys, staff satisfaction surveys, retention reports, current strategic plan status, and historical executive compensation records by the end of the first quarter of the fiscal year. 2. Executive committee prepares initial draft of performance evaluation by the end of october. 3. Executive committee presents draft review to entire board, in executive session, at annual board meeting in october/early november in order to solicit additional feedback from board members. 4. Board chair meets with ceo to discuss performance evaluation by the end of november. 5. Board chair revises, reviews, and presents final draft to executive committee during november's executive committee meeting in order to get additional feedback and recommendations. The executive committee will decide if any additional goals (outside of the strategic plan goals) need to be added to the next review timeline and will make a recommendation of any compensation adjustments. 6. The board chair presents final review and compensation recommendations to president and ceo by the end of december. 7. President and ceo provides response to review to executive committee by the end of january. 8. Ceo and executive committee meet regularly to discuss progress on strategic plan goals and any other goals set by the board. 9. Compensation changes are retroactive to january 1.

THEY PROVIDE LINKAGE TO CRISIS SERVICES FOR RESIDENTS, OTHER SERVICE PROVIDERS, LAW ENFORCEMENT, COMMUNITIES OF FAITH, BUSINESS ASSOCIATIONS, AND ELECTED OFFICIALS IN THE CITY OF CHICAGO AND ITS NORTHERN SUBURBS. CLIENTS ENTER THROUGH A CENTRALIZED POINT OF INTAKE AND ARE ASSESSED IN ORDER TO DETERMINE WHICH LEVEL OF SUPPORT WOULD BEST MEET THEIR NEEDS AND PREFERENCES. CLIENTS ARE ELIGIBLE FOR ACT IF THEY HAVE SERIOUS MENTAL ILLNESS AND EXPERIENCE THE MOST INTRACTABLE SYMPTOMS-POSING A THREAT TO THEIR ABILITY TO LIVE INDEPENDENTLY IN THE COMMUNITY. THESE INDIVIDUALS REQUIRE 24-HOUR SUPPORT AND SEVEN-DAYS A WEEK LINKAGE TO ONGOING MENTAL HEALTH SERVICES AS WELL AS PRIMARY CARE AND SUBSTANCE USE COUNSELING. CLIENTS ARE ELIGIBLE FOR CST SUPPORT IF THEY HAVE A SERIOUS MENTAL ILLNESS, A HISTORY OF CHRONIC HOMELESSNESS, REPEAT ARRESTS AND/OR INCARCERATIONS, OR EXCESSIVE USE OF EMERGENCY SERVICES WITH FAILED LINKAGES TO SERVICES. BOTH SERVICE MODELS ARE INSTRUMENTAL IN HELPING CLIENTS GAIN ACCESS TO NECESSARY RESOURCES SUCH AS MEDICAL CARE, PSYCHIATRIC SERVICES, HOUSING, AND ENTITLEMENTS, E.G., PUBLIC AID, SOCIAL SECURITY, AND MEDICAID INSURANCE.
