Form 990, Part VI, Section B, Line 12C

When an actual, potential or perceived conflict of interest is identified, a board member should summarize it at a board meeting, including any relevant steps needed to address it. This summary will be entered into the board meeting minutes, and eis council's administrative support group, benevolent vision, will insert the summary into a "register of interests" they will maintain for eis council. As a due diligence measure, the board will vote on whether the measures being taken to address such conflicts, if any, are adequate, or whether additional measures are required, and this vote will also be recorded in the minutes and noted in the register.

Form 990, Part VI, Section B, Line 15

The board of directors reviews compensation to the ceo and of the key staff. The organization follows a strict process of hiring key employees, including family members of management, for which an independent search committee (the "committee") is used. The board of directors ultimately approves or rejects the hiring of all key employees based on the recommendations from the committee.

Form 990, Part VI, Section C, Line 19

The organization makes its governing documents, conflict of interest policy, and financial statements available to public upon request.
