Activity 2

Community transformation this area of programming is funded through the federal centers for disease control and prevention with prevention and public health funds (pphf) made available through the affordable care act to reduce chronic disease. The project objectives evolved from the public health plans that were created as a result of community health needs assessments in hancock and washington counties in maine, the primary goal of the plan is chronic disease prevention, which incorporates sustainable programs for safe routes to school and access to exercise; coordinated school health; access to healthy foods and food security; access to health services and transportation; and access to dental services.

Activity 3

Food for all and farm to school this program area, funded primarily with private foundation grants and a federal community development block grant, involves a number of initiatives, including linking public schools with local growers who provide access to healthy local foods and supporting food system and nutrition curriculum integration through gardens, greenhouses and community food literacy throughout hancock and washington counties in maine. These programs also address hunger, and promote food security and nutrition education for disparate, low-income populations in hancock county, maine. Populations of focus include seniors (senior farm share), youth from 0-18 years of age, those experiencing low-income, and those eligible for supplemental nutrition assistance (supplemental nutrition assistance education program).
