Activity 2

Neighborhood networks: clc's paid community connectors organize neighborhood-widenetworks in several neighborhoods, including the oceanview-merced heights-ingleside(omi), parkmerced, st. Francis square/western addition, bayview and cayuga neighborhoods. These networks reweave communities of support focusing on food delivery, home repairs, healthy aging and breast cancer support, social and advocacy activities. Over the past year, these projects involved more than 50 volunteers, delivered food boxes to up to 140 older seniors and persons with disabilities every other week, and involved over 170 people in a series of community events and trainings. Community connectors and trained volunteers strengthen networks of friends, family and neighbors to help individuals with practical support and promote continued contribution.

Activity 3

Community networks: clc developed and hosted a series of multi-session workshops called connections for healthy aging that guide seniors that include discussions of durable power of attorney for health care, strategies for a successful patient stay and hospital discharge, staying connected on-line and much more. They showcase ways that clc community connectors have developed and maintained support networks in different communities and for individuals. This workshops have be provided in partnership with churches and other organizations. In addition, clc began the goldsmith initiative that provides practical support, assistive devices and training to seniors with vision and hearing loss. Clc is also partnering with the bay area community exchange timebank to better mobilize social capital to help individualscare for and support one another.
