Additional Explanations

Form 990, Page 1, Part I Financial Summary

Bethany on 42nd incurred financial losses in 2017 related to state Medicaid reimbursement cuts. North Dakota state law contains a provision for a budget allotment should State revenue projections fall short of anticipated expenditures for the biennium. State agencies were required to reduce general fund expenditures, and, as a result, reimbursement to Medicaid providers was reduced. The annual impact of these reimbursement cuts exceeded $1 million for Bethany facilities. While funding was partially restored in June 2017, the reimbursement cuts had a significant negative impact on financial performance for the year. In addition to reimbursement cuts, Bethany on 42nd incurred losses related to start-up of a new senior living operation, Bethany Grace Pointe. Losses were anticipated during rent-up of this new facility..

Part IV, Line 2B:

The Organization acts as custodian for the funds of the residents, as well as apartment deposits. These funds are included in cash and cash equivalents and a corresponding liability is included in accounts payable in the financial statements. These funds totaled $61,516 and $43,971 at December 31, 2017 and December 31, 2016.

Part X, Line 2:

Bethany on 42nd is a North Dakota nonprofit corporation located in Fargo, North Dakota and has been recognized by the Internal Revenue Service as exempt from federal income taxes under Internal Revenue Code Section 501(c)(3). The organization is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the organization is subject to income tax on net income that is derived from business activities that are unrelated to their exempt purpose. The organization has determined they are not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990-T) with the IRS. Bethany on 42nd believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the consolidated financial statements. Bethany on 42nd would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

Bethany on 42nd provides long term care services including skilled nursing care, transitional care services, and senior housing with services. Bethany on 42nd does not discriminate against individuals in the admissions process or in the delivery of care with regard to ability to pay for services. The Organization's cash reserves are generally applied to expansion and replacement of existing facilities and equipment, amortization of mortgages, improvement in resident care, training and education, and community benefit activities. The governing board of Bethany on 42nd is made up of prominent leaders in the Fargo-Moorhead community. Consistent with its charitable mission, Bethany on 42nd provides community benefit to the area served. In 2017, community benefit activities included:- Speaker's bureau services - provides free educational sessions to agencies and individuals in the community on topics such as: Bethany on the Grow Advance Directive EducationCareers in Long Term Care Early Dementia Care Activities with a Purpose The A, B, C & Ds of MedicareFacing Our Later Days in FaithManaging Your Resources - Estate Planning Activity With A Purpose Volunteerism - The Reward of Giving Yourself- Volunteerism - coordinates a volunteer program that included 142 volunteers giving over 10,886 hours of service, providing mental stimulation and emotional support to Bethany on 42nd's 214 residents and patients. Volunteers included members of sponsoring churches, experience works program volunteers, Moms Teach, and many student volunteers from the community's schools - elementary, middle, high schools and colleges.- School Partnerships - Bethany on 42nd works with area nursing and other healthcare-related programs to provide on-site training opportunities for students. In 2017, we had 122 nursing students complete clinical experiences at our facility. We provide a CNA training program for individuals in the Fargo-Moorhead community interested in pursuing a career as a nursing assistant. We also provide clinical site experience for an outside CNA training program and dietetic students, and internship opportunities for students in social worker programs and LTC administration. We hosted visiting students from several higher education classes. Assessment and referral services - our staff help to refer individuals needing long term care services to the appropriate agency in the community. Bethany on 42nd's licensed social workers provide guidance with medical assistance applications for residents/families. Community support - Bethany on 42nd provides meeting room space and accommodations for non-profit organizations and community groups. Several of our resident activity programs are open to community members. Facility space is open to use by outside groups for meetings, performances, activities, funeral/memorials services, music recitals, etc. Meals are provided for family members of Bethany on 42nd residents at a reduced cost. In 2017 Bethany hosted the following programs, which were open to the public: Advance Directives, talking to family about future care; Preparing for Long Term Care; and a Senior Planning Seminar, addressing Medicare, Medicaid, LTC insurance, and legal issues. Contributions were made to: First Assembly Church, Village Family Service Center, Valley Senior Services and Center for New Americans. Bethany on 42nd organizes employee participation in fundraising activities for United Way and the Alzheimer's Association. There are also representatives from Bethany on 42nd on non-profit boards and civic organizations that work to improve the quality of life in the Fargo-Moorhead area. Bethany on 42nd's President/CEO serves on the Ethos Board of Directors, and various committees of the North Dakota Long Term Care Association (NDLTCA).
