Mission

Alternatives, inc was established to provide comprehensive services to individuals/families with special needs to enable them to reach their highest level of independence and integration into the community.

Major Activities

Activity 2
The Community Outreach Services Department (COS) consists of programs dedicated to servicing individuals in both Warren and Somerset counties in recovery from mental illness, as well as homeless individuals and those with substance abuse issues. The Transitional Housing Program serves a capacity of 9 individuals coming from homeless situations, for approximately 6-12 months. During FY 12, this program served 17 individuals. Many of these individuals graduate into the Community Support Program (CSP) which serves adults with mental illness and substance abuse issues who live independently in the community. The CSP programs serve individuals in Somerset and Warren Counties, NJ on an outreach basis; wherein the clients live in either their own housing or houses owned by the agency, and receive outreach case management and clinical services such as supportive counseling, transportation, linkages to community and mental health resources, assistance with daily living skills, etc. Staff works with clients in these programs to help them recover from their mental health symptoms, gain employment, maintain their housing and finances, maintain a safe and stable residence, and improve their overall quality of life. During FY12, the Community Support Programs served 90 individuals in those two (2) counties. In addition, thirteen (13) slots in this program are set-aside for some of the most challenging individuals: Individuals who were chronically homeless prior to placement in one of our Permanent Housing programs. During FY 12, some changes were made to the structure of the COS department. CSP 3, which was formerly located in both Somerset and Warren County was split into two separate programs. CSP 3 continues to serve 15 individuals in Somerset County. The Warren County part of CSP 3, was moved into its own program- CSP 4 which serves 13 individuals. This move allowed us to increase the quality of services provided to the individuals, and have staff be more localized. Beginning in FY10 the agency received funding for the Community Supportive Living Program, located in Belvidere, Warren County, NJ. This program is funded by the NJ-Division of Mental Health and Addiction Services (DMHAS) out of an initiative put forth under the Olmstead Act, which aims to move discharge ready patients out of restrictive state psychiatric hospital settings into the community. This program provides comprehensive, 24/7 support services, to eight (8) individuals who were discharged directly from a state psychiatric hospital. Depending on the level of support needed, the consumers live in either their own 1-bedroom apartment, shared apartment with a roommate, or shared apartment with a staff office. Services are oriented towards assisting the individuals in transitioning from a hospital setting and integrating them into the community. Staff provide such services as money Management, mental health counseling, medication administration, daily living skills training, transportation, recreation events, advocacy, linkages to community resources/entitlements and vocational assistance. The overall purpose of the program is to provide a safe, stable environment for individuals to learn the skills necessary to live independently, recover from their mental health symptoms and be fully integrated back into the community. During FY12, the Community Supportive Living Program (CSL 1) served a total of 9 individuals. In FY12, the agency received funding to open a second Community Supportive Living Program (CSL 2) located in Somerset, NJ. IRS990/Description0Alternatives, Inc. Residential Programs provide people with special needs the support and training necessary to build confidence, to allow them to take charge of their own lives and ultimately attain the highest possible quality of life. In addition, the Agency provides housing and services to homeless individuals to transition into permanent housing situations. For the Fiscal year 2012 the Agency supported 593 clients with special needs through all of the programs that Alternatives provides- 17 homeless individuals through the Transitional Housing- 21 families through the Franklin House Program- 200 clients through our Bridges to Employment program- 355 clients through our Support Service Programs. In our Residential Programs Alternatives' staff provides intensive support in areas such as daily living, health management, recreation, stabilization, transportation, and money management. Services are provided 24 hours a day, 7 days a week. In addition, the Agency has a team of experienced and certified clinicians who specialize in working with people with developmental disabilities and mental health issues. The Clinical Team addresses behavioral issues, emotional disorders, substance abuse issues, or a combination of these diagnoses. The comprehensive approach coincides with the person's goals and the agency's person- centered philosophy. The residential programs include Group Homes, Supervised Apartment Programs, Supported Living Programs, Independent Living, Parenting Program, Supportive Housing, Transitional Housing, Community Supportive Living (CSL) and our Emergency Capacity Systems Program, depending on the level of assistance needed by an individual. Individuals are referred to the Agency through various state and local agencies, including the NJ Department of Development Disabilities, Division of Mental Health & Addiction Services and county social service agencies. For the Fiscal year 2012 the Agency was successful in: Expanding our Group Home programs to include an additional home in Ocean County, New Jersey that services four (4) individuals. Opening a new Community Supportive Living (CSL) program that services 4 clients from state psychiatric hospitals through the Olmstead Act. Helping stabilize 12 individuals through our Emergency Capacity program and locating permanent program placement for them. Transitioning 2 families from our Supervised Parenting Program to our Supportive Living Program, helping them become more independent and enabling them to move to a less supervised program environment.
