OTHER PROGRAM SERVICES 4: Outreach, Enrollment and Access - One very important strategy to support our mission is to get people who qualify for coverage enrolled and able to use their health insurance. In February 2014 we began piloting a Premium Assistance Program. Premium Assistance participants earn below 200% of the Federal Poverty Level, which previously qualified them for 100% discounted services with hospital systems and other health care providers. As of January 1, 2014, however, they are required to pay premiums, deductibles, co-insurance, and prescription costs, placing private insurance out of reach for many of them. Project Access NOW leverages the tax credits and cost sharing reductions available to participants to pay the balance of the premiums for the medical and dental plans they have selected. Our volunteer network supports program participants by writing off the remaining out of pocket expenses, which are often a bigger barrier than the premium payments themselves. 363 people were enrolled in Premium Assistance during this fiscal year. Starting in February of 2015 we began a pilot project to provide health insurance outreach and enrollment activities targeting low-income families. Now we are able to combine many of our strategies to make sure that everyone we touch gets access to the care they need whether it is through Medicaid, the Health Insurance Exchange or our Classic Program. From February 2015 to June 2015, 2,235 people were enrolled in health insurance coverage. Outreach and Enrollment Assisters knocked on over 54,000 doors in low income neighborhoods and nearly 40,000 homes in those areas received direct mail information about insurance enrollment.

Part X : FIN48 Footnote

The Organization could be subject to income tax on unrelated business income. The Organization's federal and state information returns are subject to possible examination by the taxing authorities until the expiration of the related statutes of limitations on those information returns. In general, the federal and state information returns have a three year statute of limitations. Should that status be challenged in the future, the Organization's 2013, 2012 and 2011 tax years are open for examination by the Internal Revenue Service. The Organization would recognize accrued interest and penalties associated with uncertain tax provisions, if any, as part of the income tax provision.

With the C3 Community Assistance Program, we support discharge planners, social workers, and care managers to connect low-income patients to the resources they need for a safe discharge. These resources may include transportation, temporary housing, and medication assistance. Providence Health & Services has been our development partner with this initiative and we expect expansion to other hospital systems. IRS990/ProgSrvcAccomActy3Grp/Desc0Donated Care Coordination Services - Nationally known as the Pathways Community HUB, the Pathways model focuses on connecting people in need to critical resources and paying for the outcome of actually connecting to a resource, not just making a referral. For over three years, Project Access NOW has been working with a number of social service organizations in the Rockwood neighborhood of East Multnomah County. Rockwood has a great richness in racial, ethnic, and religious diversity, but it is a highly distressed area with increasing crime, dense poverty, and decreasing success in education, employment, and access to healthy food. Partner organizations in the Rockwood Pathways Project are committed to working collaboratively to improve the health of this community by creating a cross-sector system that holistically meets the needs of a family or individual through supported, equitable, and appropriate connection to care and services. The data collected is then used to identify system inequities and enable the community to make the changes needed to ensure our systems benefit the people they serve. From March June 2015 we have been building the infrastructure to launch this project in the fall of 2015. A smaller Pathways Project known as Screen and Intervene has connected families with children from Providence Health & Service Residency Clinics to a variety of resources to address food insecurity. 45 children and their families were connected to food, social service and other resources during this fiscal year.
