Pharmacy - A core piece of health care is access to needed medication to manage chronic conditions and cure episodic conditions. Pharmacy Bridge Provides patients with basic medications for a $4 co-pay. In addition, the medication is available directly through the pharmacy benefit. We also use volunteers to process medication assistance applications for patients to receive brand name medications at no cost. The contracted rate for medications made available to us saves over 60% off of the average wholesale price of medication. During this fiscal year, Project Access NOW filled over 8,229 prescriptions though pharmacies, paying $109,862 for mediations that would have retailed at $326,238.

Activity 3

Coordination of volunteer Network - We are able to connect people to care as long as there are health care providers willing to donate that care. Our network of volunteer providers grew from 2,800 to nearly 2,900 this year. This network includes all of the hospitals in the metro region and several private specialty care providers. Supporting and coordinating this network requires a sophisticated database, recruitment efforts, volunteer support and a great deal of relationship building and trouble shooting. One core piece of this is managing equity between hospital system partners. Another key piece is assuring the Care Coordination efforts are successful in order to keep volunteers involved. Over 99% of our volunteers that have signed up to provide volunteer care through Project Access NOW have continued with their participation; some for nearly five years now!
