Governance Explanations

Form 990, Part VI, Section B, line 11B

Form 990 is prepared by an independent CPA firm and is reviewed in detail by the organization's Treasurer and Secretary for accuracy. The 990 is then reviewed by the organization's board of directors prior to being filed with the IRS.

Form 990, Part VI, Section B, line 12C

In addition to the annual review and signing of the policy, board members and officers are required to bring any potential conflicts that arise over the course of the year to the attention of the President and Executive Director or Board Chair. If a conflict of interest is determined to exist, the board member with the conflict would recuse him/herself from any board decision-making process, and the recusal would be reflected in the meeting minutes.

Form 990, Part VI, Section B, line 15

15a - The board of directors determine appropriate compensation for the organization's President and Executive Director. In determining compensation for the President, comparability data is referenced. The board of directors review the comparability data, and discuss and approve compensation. Compensation decisions are documented in the board minutes. 15b - The compensation for other officers and senior staff is determined on an annual basis by the President and Executive Director using comparable data. The deliberation and decision process is documented.

Form 990, Part VI, Section C, line 19

The organization makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.

IRS990/Desc0In 2019, Phoenix Multisport, Inc. (dba "The Phoenix") began expanding our capacity, through additions in technology and staff, to increase our ability to serve one million people with free recovery support in the next five years. Additionally, we continued to grow programs and attendance in our brick-and-mortar locations in California, Colorado, Massachusetts, Idaho, and Billings, MT. We also expanded our brick-and-mortar operations by integrating the programs and operations of Root to Rise, in Belmar, NJ and built a new facility in Wichita, KS. Further, the Phoenix expanded volunteer led programming sites across the nation and was operating in 46 cities at the end of 2019. The Phoenix's sober active community reached 11,530 new individuals in 2019. (Continued on Sch. O) Peer recovery support programs offered by The Phoenix help people stay engaged in the recovery process and reduce the likelihood of relapse among those with substance use disorders. Colorado update: Volunteer-led locations brought Phoenix programs to new communities in Fort Collins and Steamboat. Brand awareness and individual donations increased with the launch of peer-to-peer fundraising events such as a springtime Ragnar race in Aspen and an August fitness competition in Colorado Springs. Programs like mindfulness and art classes were added to the calendar in Denver, so that low-barrier events are always available at Phoenix. California update: Free community programs continue to grow as well as partnerships with treatment centers and other community organizations, serving over 800 individuals in 2019. Phoenix Orange County hosted their 3rd annual Phoenix Unbroken fitness competition-fundraiser with a record turnout of athletes from local gyms, and won first-time grant awards from 3 local foundations.Massachusetts update: In 2019 The Phoenix served over 2,000 individuals throughout Massachusetts. This was largely a result of partnerships with other impactful organizations throughout the state including the Middlesex County Sheriff's Office, The Gavin Foundation, Federal Probation Reentry Services, the Rose Kennedy Greenway and several recovery resources and treatment centers. The Phoenix also formed a partnership with The Boston Red Sox, which allowed us to highlight our programs on a larger scale, helping to reduce the stigma around substance use and recovery. This partnership celebrated some of our team members on the field and our Founder threw the first pitch.Additionally, The Phoenix shifted away from our Per Ignem membership-based strategy. This shift allowed The Phoenix to put all time and talent towards expanding our reach throughout Massachusetts and building our base of volunteers, this included launching programs in Bourne, MA.Boise update: The Phoenix in Boise served close to 600 members and built collaborative relationships with many Idaho organizations like IDOC, The VA, and VCORP. Our impact spread to areas across the state, including Sun Valley and North Idaho. We increased class offerings and social events allowing for more awareness and engagement of our community.Montana update: Billings entered it's second year of programing in 2019, serving 183 team members and hosting 309 Phoenix events. Phoenix Montana partnered with a Drug Court to host a community event " Walk for Recovery".New Jersey update: The Phoenix launched in New Jersey in January 2019 by integrating the programming and operations of Root to Rise. With 11 paid contracted partnerships, the phoenix was able to offer community and private programming by hiring 6 part-time instructors, with additional volunteer led programs throughout the state.Wichita update: In 2019, the Phoenix began construction on a 9,000+ sq ft facility in Wichita, KS to expand our community led programming in the Midwest.National expansion efforts: The Phoenix continued its expansion through the launch of volunteer-led remote programs across the country. By the end of 2019, Phoenix programs were in 22 stat
