IS COMPOSED OF TWO CLINICS AIMED TO MEET THE MEDICAL CARE AND SOCIAL SERVICE NEEDS OF HOMELESS FAMILIES AND INDIVIDUALS. BERGEN LANNING HEALTH CENTER, PROJECT H.O.P.E., INC.'S "MEDICAL HOME" FOR THE HOMELESS, IS OPEN FIVE DAYS A WEEK AND IS STAFFED BY A NURSE PRACTITIONER, TWO PHYSICIANS, INCLUDING A FULL-TIME MEDICAL DIRECTOR, A NURSE CLINICAL COORDINATOR, A LICENSED PRACTICAL NURSE, A SOCIAL WORKER/CASE MANAGER, A DRUG AND ALCOHOL COUNSELOR, TWO FINANCIAL COUNSELORS, AND TWO MEDICAL RECEPTIONISTS. PRESENTLY, THE CENTER SEES AN AVERAGE OF 100 HOMELESS CLIENTS PER WEEK. IN ADDITION TO PRIMARY HEALTH SERVICES AND MENTAL HEALTH AND SUBSTANCE ABUSE COUNSELING, PRESCRIPTION ASSISTANCE, CASE MANAGEMENT, EMPLOYMENT, HOUSING, FINANCIAL COUNSELING IS AVAILABLE AT THE CENTER. PROJECT H.O.P.E., INC. IS ONE OF THE FIRST MEDICAL PRACTICES IN CAMDEN TO IMPLEMENT ELECTRONIC MEDICAL RECORDS TO PROMOTE QUALITY CARE. PROJECT H.O.P.E., INC. IS NO LONGER USING PAPER CHARTS AND IS CURRENTLY SENDING PRESCRIPTIONS TO PHARMACIES ELECTRONICALLY. PROJECT H.O.P.E., INC. ALSO CONDUCTS A SATELLITE CLINIC ONE DAY A WEEK AT THE VOLUNTEERS OF AMERICA ALETHA WRIGHT TRANSITIONAL LIVING PROGRAM LOCATED IN CAMDEN, NEW JERSEY. THIS CLINIC PROVIDES INFECTIOUS DISEASE SCREENING, PRIMARY HEALTH CARE, CASE MANAGEMENT, AND FINANCIAL COUNSELING SERVICES TO RESIDENTS OF THE ALETHA WRIGHT TRANSITIONAL LIVING PROGRAM. STAFF AT THE SATELLITE CLINIC INCLUDES TWO MEDICAL PROVIDERS, INCLUDING ONE VOLUNTEER PHYSICIAN AND ONE VOLUNTEER NURSE PRACTITIONER, A LICENSED PRACTICAL NURSE, AND A SOCIAL WORKER.
