Management reviews all questions and prepares the documentation for the form 990. The 990 is then submitted electronically to the board of directors for comment. Should no comments be forthcoming, the 990 will be signed and submitted.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is part of the personnel policy manual, which is issued annually to all employees and to new employees upon hire during orientation. Employees are required to read and sign off that they have received and read the personnel policy manual. Program directors and above are sent a conflict of interest questionnaire annually and they are required to complete the form and submit it to the chief human resources officer.

Form 990, Part VI, Section B, Line 15A

In july, 2021, the agency has used information gathered in the 2020 compensation study to create and rollout an agency-wide compensation plan establishing pay best practices within the confines of our respective contracted budgets. The plan establishes a framework for how pay for new hires, promotions, demotions, and performance-based merit, which includes a merit matrix based on evaluation rating and compa-ratio as well as a new hire and promotion matrix that offers guidance appropriate offer level based on job-related education and experience to ensure consistency and fair pay practices. Since the rollout of the plan, csi has addressed all green-circled incumbent pay by systematically increasing pay to within the respective position paygrades, factoring in incremental increases based on service time. The agency also addressed a handful of red-circled incumbent pay by providing a lump-sum performance-based merit payment.

Governing documents are available upon request. The conflict of interest policy is part of the personnel policy manual which is issued to new employees upon hire and on an annual basis thereafter. Annually, following the board of director's approval, the audited financial statements are submitted to funding sources and the bank and are made available upon request. MST INTERVENTIONS ADDRESS THE MANY FACTORS THAT ARE KNOWN TO CONTRIBUTE TO DELINQUENCY WHERE THE YOUTH LIVES, WORKS, AND PLAYS. CSI HAS BEEN DELIVERING MST SERVICES FOR 24 YEARS AND CURRENTLY PROVIDES SERVICES IN GA, KS, MD, NH, NJ, AND PA. THE FY2023 DISCHARGE DATA SHOWS: YOUTH LIVING AT HOME - 96%; STILL IN SCHOOL AND/OR AN APPROVED VOCATIONAL PROGRAM - 88%; AND YOUTH WITH NO NEW ARREST - 88%.

AICS ARE OPEN FOR EXTENDED DAY, EVENING, AND WEEKEND HOURS. THE AIC STAFF MEMBERS ADMINISTER ASSESSMENTS, USE TARGETED INTERVENTION STRATEGIES, ASSIST CLIENTS IN GAINING FULL-TIME EMPLOYMENT, AND HELP MEET OTHER INDIVIDUALIZED CLIENT NEEDS. CSI HAS BEEN PROVIDING AIC SERVICES FOR MORE THAN 30 YEARS AND IS THE LARGEST AIC PROVIDER IN CONNECTICUT. DURING FY2023, THE AICS HAD A TOTAL OF 1,971 ADMISSIONS AND ACHIEVED AN 85% SUCCESSFUL COMPLETION RATE.
