Governance Explanations

Form 990, Part VI, Section B, Line 11B

Management reviews all questions and prepares the documentation for the form 990. The 990 is then submitted electronically to the board of directors for comment. Should no comments be forthcoming, the 990 will be signed and submitted.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is part of the personnel policy manual, which is issued annually to all employees and to new employees upon hire during orientation. Employees are required to read and sign off that they have received and read the personnel policy manual. Program directors and above are sent a conflict of interest questionnaire annually and they are required to complete the form and submit it to the chief human resources officer.

Form 990, Part VI, Section B, Line 15A

In 2020, csi hired the employer's association of the northeast (eane) to conduct a compensation study. Utilizing the national position evaluation plan, all jobs were objectively evaluated on compensable factors including knowledge, experience, physical demand, mental/visual acuity, working conditions, supervision received and supervision of others. Once evaluated, jobs were placed in labor grades based on their respective scores, benchmark positions were identified and a market analysis was complete using the most recent salary data from non-profit salary surveys including the non-profit times (bluewater), payfactors, third sector new england and eane's own compensation survey. In addition, the guidestar report was purchased and consulted. Incumbent compensation has been evaluated against the minimum, median, and maxium points for the respective salary grade in which the incumbent's position landed. The agency is currently addressing any red-circled and green-circled incumbent pay.

Governing documents are available upon request. The conflict of interest policy is part of the personnel policy manual which is issued to new employees upon hire and on an annual basis thereafter. Annually, following the board of director's approval, the audited financial statements are submitted to funding sources and the bank and are made available upon request. Form 990 is also made available through guidestar.org.

CSI HAS BEEN DELIVERING MST SERVICES FOR 22 YEARS AND CURRENTLY PROVIDES SERVICES IN GA, KS, MD, NH, NJ, AND PA. THE FY2021 DISCHARGE DATA SHOWS: YOUTH LIVING AT HOME - 90%; STILL IN SCHOOL AND/OR AN APPROVED VOCATIONAL PROGRAM - 82%; AND YOUTH WITH NO NEW ARREST - 91%.
