Governance Explanations

Form 990, Part VI, Section B, Line 11

The 990 is reviewed by management to verify amounts tie to internal financial records and to the audited financial statements. Management reviews all questions and prepares documentation for the form 990. The 990 is then submitted electronically to the audit committee and chair of the board of directors. The audit committee chair will then submit the 990 to the full board for review and allow a period of time for feedback and questions. The audit committee chair will then notify the ceo and cfo when it is appropriate for the 990 to be submitted.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy is part of the personnel policy manual, which is issued annually to all employees and to new employees upon hire during orientation. Employees are required to read and sign off that they have received and read the personnel policy manual. Program directors and above are sent a conflict of interest questionnaire annually and they are required to complete the form and submit it to the h.r. Manager.

The human resource manager annually participates in and purchases surveys to validate company compensation practices. During fiscal year 2014 she purchased the 2013 edition of the non profit times salary & benefits report.

Form 990, Part VI, Section C, Line 19

Governing documents and the conflict of interest policy are made available upon request. Annually, following the board of directors approval, the audited financial statements are submitted to funding sources and the bank and are made available upon request. Form 990 is also available through guidestar.org.

Financial Statement Notes

PART X, LINE 2:

Tax positions are taken based on interpretation of federal and state income tax laws for which the outcome is uncertain. Management periodically reviews and evaluates the status of uncertain tax positions. Changes may result from closing of statute of limitations on tax returns, new legislation, and clarification of existing legislation through government pronouncements, the courts and through the examination process. The policy is to report interest and penalties, if any, in unrelated business income tax expense in the statement of activities. No interest or penalties have been accrued or charged to expense as of june 30, 2014 or for the year then ended. As of june 30, 2014, the tax years subject to examination are 2010 through 2012, with the 2013 return to be filed.

DURING FY2014, THE FOLLOWING SUCCESS RATES WERE ACHIEVED: THE BERMAN TREATMENT CENTER-96%, THE FEMALE WORK RELEASE/REENTRY PROGRAM AT HARTFORD HOUSE-79%, THE CHEYNEY HOUSE-59%, WATKINSON HOUSE-80%. MST VIEWS AN INDIVIDUAL'S BEHAVIOR AS BEING INFLUENCED BY THE SYSTEMS AROUND THEM, FAMILY, PEERS, SCHOOL, AND NEIGHBORHOOD. MST INTERVENTIONS ADDRESS THE MANY FACTORS THAT ARE KNOWN TO CONTRIBUTE TO DELINQUENCY WHERE THE YOUTH LIVES, WORKS AND PLAYS. CSI HAS BEEN DELIVERING MST SERVICES FOR THIRTEEN YEARS. WE CURRENTLY DELIVER SERVICES IN CT, GA, MD, NJ, NY, PA AND RI. THE 2014 DISCHARGE DATA SHOWS: YOUTH LIVING AT HOME-87.6%, STILL IN SCHOOL-85.9%; YOUTH WITH NO NEW ARREST-86.2%. NATIONAL AVERAGE OF MST TEAM DATA: YOUTH LIVING AT HOME-86.2%, YOUTH STILL IN SCHOOL-86.6% AND YOUTH WITH NO ARRESTS-85.6%.

FUNDED BY THE CONNECTICUT JUDICIAL DEPARTMENT COURT SUPPORT SERVICES DIVISION (CSSD), THESE PROGRAMS ACCEPT COURT-REFERRED CLIENTS AS PART OF A PRETRIAL DIVERSION PROGRAM. THEY ALSO PROVIDE SERVICES TO PROBATION AND PAROLE PRE-VIOLATORS. THE PROGRAMS ARE OPEN FOR EXTENDED DAY, EVENING AND WEEKEND HOURS. THE AIC STAFF MEMBERS ADMINISTER ASSESSMENTS, USE TARGETED INTERVENTION STRATEGIES, AND ASSIST CLIENTS IN GAINING FULL-TIME EMPLOYMENT. CSI HAS BEEN PROVIDING AIC SERVICES FOR OVER 20 YEARS AND IS THE LARGEST AIC PROVIDER IN CONNECTICUT. DURING FY2014, AIC HAD A CUMULATIVE AVERAGE UTILIZATION OF 3348 (ROUGHLY 279 PER MONTH) AND ACHIEVED AN 86% SUCCESSFUL COMPLETION RATE.

MULTIPLE SERVICES ARE PROVIDED SUCH AS LIFE SKILLS, GED CLASSES, EMPLOYMENT READINESS, SUBSTANCE ABUSE EVALUATION AND TREATMENT, PARENTING, ANGER MANAGEMENT, COGNITIVE BEHAVIORAL THERAPY, COMPUTER SKILLS, FAMILY THERAPY, HOUSING ASSISTANCE, MENTAL HEALTH REFERRALS AND MOTIVATIONAL INTERVIEWING. CSI ALSO OPERATES TWO DRC PROGRAMS IN SANTA BARBARA AND SANTA MARIA, CALIFORNIA, FUNDED BY THE SHERIFF'S DEPARTMENT, FOR PAROLE CLIENTS. CSI PROVIDES TREATMENT TO PROBATION CLIENTS IN THESE LOCATIONS UNDER CONTRACT WITH THE ADULT PROBATION DEPARTMENT. THIS MODEL INCLUDES MANY OF THE SAME SERVICES ABOVE PLUS COMMUNITY SERVICE OPPORTUNITIES. DURING FY 2014, THE FOLLOWING SUCCESS RATES WERE ACHIEVED: NJCRC-82%, DRC-57% AND PROVIDED 4,527 HOURS OF COMMUNITY SERVICE. CSI ALSO ADDED SAN BERNARDINO COUNTY PROBATION TO THE POPULATION SERVED IN SANTA BARBARA AND SANTA MARIA.
