Mission

Elderhouse promotes the health and wellbeing of aging adults through a holistic medical model adult day program led by a passionate team of therapeutic recreation professionals, health aides, and nurses specially trained to support the unique needs of each individual. At the center, we foster an environment of inclusivity and enrichment through activities centered on celebrating personal successes. We ensure safety through personal attention and dependable door-to-door transportation. We build trust with families, offering caregivers respite from caregiving duties and peace of mind knowing their loved ones are in our care.

Elderhouse adult day center is committed to providing aging seniors with a safe, nurturing environment where they can share time with others while receiving the care they need. Our accredited medical model program is designed to improve quality of life by helping seniors remain as independent as possible and to provide caregivers peace of mind.

Governance Explanations

Form 990, Part VI, Section B, Line 11B

Elderhouse, inc. Has its form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reported is complete and accurate. Prior to filing, the form 990 will be sent to the president of the board of directors who, along with the executive director and the executive committee, will review the document and sign it. Once the executive committee has approved the return it is filed with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

Prior to board or committee action on a contract or transaction involving a conflict of interest, a trustee or committee member having a conflict of interest and who is in attendance at the meeting shall disclose all facts material to the conflicts of interest. Such disclosure shall be reflected in the minutes of the meeting. A trustee or committee member who plans not to attend a meeting at which he or she has reason to believe that the board or committee will act on a matter in which the person has a conflict of interest shall disclose to the chair of the meeting all facts material to the conflicts of interest. The chair shall report the disclosure at the meeting and the disclosure shall be reflected in the minutes of the meeting. A person who has a conflict of interest shall not participate in or be permitted to hear the board's or committee's discussion of the matter except to disclose material facts and to respond to questions. Such person shall not attempt to exert his or her personal influence with respect to the matter, either at our outside the meeting. A person who has a conflict of interest with respect to a contract or transaction that will be voted on at a meeting shall not be counted in determining the presence of a quorum for purposes of the vote. The person having a conflict of interest may not vote on the contract or transaction and shall not be present in the meeting room when the vote is taken, unless the vote is by secret ballot. Each new responsible person shall be required to review a copy of this policy and to acknowledge in writing that he or she has done so. Each responsible person shall annually complete a disclosure form identifying any relationships, positions or circumstances in which the responsible person is involved that he or she believes could contribute to a conflict arising. Such relationships, positions, or circumstances might include service as a director of or consultant to a nonprofit organization, or ownership of a business that might provide goods or services to elderhouse. Any such information regarding business interests of a responsible person or a family member shall be treated as confidential and shall generally be made available only to the president and executive director, and any committee appointed to address conflicts of interest, except to the extent additional disclosure is necessary in connection with the implementation of the policy. This policy shall be reviewed annually by each member of the board of directors. Any changes to the policy shall be communicated immediately to all responsible persons.

Form 990, Part VI, Section B, Line 15A

Responsibility to establish fair and equitable compensation for the executive director rests with the president of the board of directors and an ad hoc compensation committee appointed by the president. The president shall appoint to this committee only independent board members. The review of the executive director will be conducted under these guidelines: 1. The review will be conducted annually at fiscal year end 2. The review takes into account compensation history 3. The committee reviews the most available comparative data 4. The review results in a report that is delivered to the full board 5. The review is concluded with documentation of all actions added to a personnel file

To serve the growing number of individuals taking on the role of caregiving, elderhouse is now an accredited provider of adult family living services. The new adult family living program offers guidance and financial support to caregivers, helping them to succeed in providing the best care possible at home. Caregivers receive expert advice on managing the daily tasks associated with caring for an aging adult.

Elderhouse, inc. Ceased operations of their community transportation program. Elderhouse provided transportation to seniors and disabled adults, totaling 1,200 one way rides to medical appointments, employment, education classes, shopping, and social activities throughout fairfield county under the americans with disabilities act.
