Founded in 1989 to provide prenatal care for lowincome and homeless women, HPP has evolved into a dynamic family resource center with a holistic model that integrates a wide array of services to address the specific needs of each client coordinated by a team of family case managers (~50% of whom have similar life experiences as the clients we serve.) HPP offers support for families by offering a 3-pronged menu of services. Families seeking support at HPP matriculate into Housing Assistance Family Support Services and/or wellness services. Most of these services are currently being offered in a remote capacity.
In partnership with our clients,break the cycle of childhood poverty. Through supportive,nonjudgmental case management, we empower families motivated by pregnancy and parenthood to recognize their strengths and trust in their own capacity to transform their lives

“OTHER PROGRAM SERVICES 4: Housing Assistance Program HPP provides limited, short-time financial assistance (rental deposits and bill assistance) that helps families obtain and retain safe and permanent housing. Weekly workshops educate clients on the rights and responsibilities of renters. The agency also provides limited long-term subsidies for a select number of high-need families. OTHER PROGRAM SERVICES 5: Wellness Center HPP offers prenatal classes, post-partum support groups, doula (birth coach) support, alternative health services (e.g., massage, yoga, and acupuncture), parenting workshops, group prenatal care and family planning counseling designed to help women deliver and raise healthy babies. The Wellness Center also offers family planning education and support. New Beginnings, a program within the Wellness Center, is an intensive outreach project targeting pregnant women at-risk of difficult pregnancies and birth complications, due to a variety factors including but not limited to substance abuse, mental health issues, homelessness and other factors. OTHER PROGRAM SERVICES 6: Auxiliary ServicesAs a family resource center, HPP provides integrated services that help families achieve long-term success. Case managers anchor HPPs service delivery and build relationships with clients. Case managers work with clients to resolve crisis situations so that goals related to long-term self-sufficiency can be met. Other HPP support services include: The Childcare Center provides quality childcare for children while their parents participate in services. Trained in child development, Childcare Center staff facilitates age-appropriate activities for participants, with a recent emphasis on preparing younger children to be school ready. The Childcare Center also assists HPP clients with applications to the San Francisco Child Care Connection (SFC3) list for free high-quality pre-schools and subsidized day care.The Family Economic Success program offers free tax preparation services, education in the U.S. Tax Code, and assistance with IRS initiated tax issues; assistance in obtaining public benefits to which clients are entitled (e.g., food stamps, health care); and financial education that covers banking, budgeting, credit building, credit reports and responsible credit use, and an introduction to loans and home ownership. The Community Technology Center (CTC) provides clients the opportunity to learn computer usage and literacy, establish email accounts, attend classes on Microsoft Office applications, conduct job and housing searches, and participate in online social networks, news, and media. HPP volunteers provide one-on-one and group tutoring; help with job searches and assistance in preparing resumes.Mental Health Services provide clients with individual, couples, dyadic (parent/child), family and group psychotherapy, as well as clinical consultation for staff. Mental Health staff also lead various child development activities for parents and children. Fatherhood Support services provide group and one-on-one support specially designed for fathers. Enrichment Classes such as sewing, knitting, ESL (English as a Second Language) and art classes help clients learn valuable, therapeutic skills while their group format encourages community-building.Referrals and linkages to dozens of other social service agencies for services HPP is unable to provide (e.g., substance abuse treatment programs, legal services). Case managers coordinate these referrals for clients as part of their overall plan for self-sufficiency. These agencies refer their clients to HPP to maximize the value of the services clients receive.”

| IRS990/Desc | 0 | Child Welfare Services Keeping Families Together (KFT), San Francisco Family Treatment Court (FTC) and Families Moving Forward (FMF) constitute HPPs Child Welfare Services. - KFT is a program through which San Franciscos Child Protective Services (CPS) and HPP work with families to keep them unified and prevent children from entering into the CPS system because of substance abuse issues in the family. The Peer Mentor Program is a part of KFT, wherein HPP staff members (who previously have had involvement with CPS) work closely with clients as they satisfy the requirements of the court for family reunification. Peer Mentors lead weekly support groups, accompany clients to court and medical appointments, and steward clients to appointments and other obligations. - FTC is a court-supervised treatment and parenting program for people with children involved in the dependency system. A collaborative justice program, the FTC is a partnership between Human Services Agency, the Department of Public Health, the City Attorneys Office, the panel of dependency attorneys, the Homeless Prenatal Program, and the Superior Court. HPP is also part of a city-wide Substance Abuse Services collaborative, serving as a link between the streets and drug treatment programs for pregnant and parenting mothers with substance abuse problems. - FMF was established with the support of a federal demonstration grant (administered through the City of San Francisco), FMF uses a housing first approach, working with carefully selected, highest-need homeless families to help them obtain and maintain stable housing, providing a foundation for stability. Once families are securely housed, they receive case management services, job training, parenting classes, and life-skills counseling for greater self-sufficiency. |
