Mission

The mission of hope hospice, inc. Is to provide high quality medical care and emotional support for terminally ill patients and their families/caregivers.

Hope hospice, inc. Was incorporated in 1980 to provide specialized medical care and emotional support for terminally-ill patients and their families. In 2015-16, the organization continued its geographic service area to include all communities north of the tri-valley to walnut creek, pleasant hill and concord, as well as the communities of san lorenzo and hayward to the west. The transitions program continues to support the needs of pre-hospice patients, as well as those whose health improves and "graduate" from service. Families need help with community resources during this transitional period of their lives when they do not meet criteria for hospice care. Hope hospice provided community outreach activities during the year to promote advance directives, care giving support, grief support and hospice care. Hope hospice partnered with patient facilities, valleycare medical center, valley humane society, county of alameda, accma.

All individuals who are stakeholders (including employees and directors) must execute a conflict of interest disclosure statement. These are completed upon hire of employees/contract employees or board approval in the case of directors, and renewed/updated on an annual basis. All actual or apparent conflicts of interest by individuals of hope hospice shall be disclosed promptly and fully to the ceo and/or the board of directors. Such disclosure shall be in writing and kept on file by hope hospice. Individuals having such conflict may be prohibited from specific involvement in areas of conflict. The prohibition shall vary according to the seriousness of the conflict from mere abstention from voting on an issue to termination of employment or revocation of board director status.

Financial Statement Notes

PART X, LINE 2:

Hope hospice is recognized as a public charity exempt from federal income tax under section 501(c) (3) of the internal revenue code and section 23701(d) of the california revenue and taxation code, whereby only unrelated business income, as defined by section 512(a)(1) of the internal revenue code and similar code section of the california revenue and taxation code, is subject to income tax. Home health is a pass-through entity for income tax reporting purposes and, accordingly, does not pay tax on its income. Instead, income or loss is reported on the returns of hope hospice. Each year management considers whether any material tax positions the organization has taken are more likely than not to be sustained upon examination by the applicable taxing authority. Management believes that any positions the organization has taken are supported by substantial authority and, hence, do not need to be measured or disclosed in these consolidated financial statements.

PRESENTATIONS WERE MADE AT COMMUNITY ORGANIZATIONS, LOCAL BOARD AND CARE HOMES, SKILLED NURSING FACILITIES, OTHER MEDICAL CARE PROVIDERS, AND AT HOPE HOSPICE OFFICES AND BUSINESSES. HOPE HOSPICE ALSO PROVIDES VALUABLE TRAINING FOR HOSPICE VOLUNTEERS WHO SUPPORT THE SERVICES WE OFFER. VOLUNTEERS ARE KEY COMPONENT OF HOSPICE CARE; THUS, RECRUITMENT , RETENTION, AND TRAINING OF VOLUNTEERS IS CRITICAL. THE GREIF SUPPORT ALSO HOSTED A VARIETY OF GRIEF AND LOSS WORKSHOPS FOR THE COMMUNITY, INCLUDING MANAGING GRIEF DURING THE HOLIDAYS, PET LOSS SUPPORT, LOSS OF SPOUSE, AND OTHER TOPICS RELATED TO HEALING.
