Activity 2

Medical & community outreach program - between july 1, 2012 and june 30, 2013, hope hospice clinicians provided in-service trainings to nursing staff at 5 skilled nursing facilities and 15 residential care facilities for the elderly. In addition, we visited 76 area physicians to share the latest information of how to an appropriate referral to a hospice.members of our medical and community attended and exhibited at 7 community events and health fairs in castro valley, danville, livermore, pleasanton, and san ramon. Furthermore, our volunteer speakers bureau made presentations to 26 civic, business, and faith-based organizations. Our 2nd annual "the hike for hope" spring memorial hike and fundraising event attracted more than 750 hikers, and the "lights of the valley" holiday memorial attracted more than 250 area residents. We hosted the annual thanksgiving memorial held for 60+ grieving families.

Activity 3

Adult & children's grief support program - hope hospice provides grief support and education for the family/caregiver for 13 months following the death of a patient. Support groups and services for children and teens who have suffered a loss in their family are also available. During this reporting period, we served 1,259 adult clients and 173 children. Hope hospice also provides grief support groups and grief counseling for members of the general community.
