Governance Explanations

Form 990, Part VI, Section A, Line 2

Dianne larsen and jason slye have a family relationship.

Form 990, Part VI, Section A, Line 4

At the end of 2013, the organization revised its mission statement, but there have been no changes to the program activities.

Form 990, Part VI, Section B, Line 11

The finance manager and executive director ("ed") review the draft 990 initially, and then a copy is sent to the finance committee, and then to all board members for review prior to filing.

Form 990, Part VI, Section B, Line 12C

Employees, board members and volunteers are covered under this policy. The executive director will provide an annual questionnaire to board members and senior staff in order to identify areas that could give rise to conflicts of interest. Board members and senior staff will promptly update any disclosures previously made. The decision around any conflict of interest brought to the board will be documented in the minutes of the board meetings.

Form 990, Part VI, Section B, Line 15A

Compensation for all agency positions is considered using a three-county salary survey which has been conducted by united way every two years. The last survey was done in early 2013. The next edition will be published by washington employers. In 2013, all employees except the executive director were given a 2% cola. A board committee completes a performance review of the executive director annually and the board approves any salary adjustments for the executive director.
