Its clients to become self sufficient once they leave the program. Help's path program serves the severely mentally ill clients. Path has been serving clients since march 2013. These clients are connected with service to stabilize their mental health issues and address other barriers that have lead to their homelessness. A new start program (ans) has been providing housing and case management to families and couples since march 2012. Ans is a supportive housing program that provides rental assistance and supportive services to homeless families/couples who have a documented disability. The program focuses on providing case management, programming and support to its clients to become self sufficient once they leave the program. -number of clients who participated in the program year-new beginnings - 212, help them home -62, outreach - 569, path - 366, ans - 161. -number of clients discharged throughout the program year: new beginnings - 83, help them home - 23, outreach - 540, path - 244, ans - 54. -number of clients brought on to caseload: new beginnings - 212, help them home - 62, outreach -29, path - 219, ans - 161. -number of active clients at the end of the program year: new beginnings - 138, help them home - 33, outreach - 29, path - 46, ans - 39. -percentage of clients with substance abuse issues: new beginnings - 134 or 63.2%, help them home - 46 or 74.2 %, outreach - 218 or 38.2%, path - 242 or 66.1%, ans - 97 or 60.3% -percentage of clients with stable exits from the program: new beginnings - 33 or 15.5%, help them home - 6 or .06%, outreach - 29 or 1 %, path - 43, ans - 6 -percentage of clients who are veterans: new beginnings - 12or 5.7%, help them home - 3 or 4.7%, outreach - 32or 5.6%, path - 37 or 10.1%, ans - 1 or 1%. Community outreach help holiday programs consist of thanksgiving assistance, holiday toy assistance, and adopt-a-family. Clients receiving thanksgiving assistance receive a frozen turkey and groceries to prepare a traditional thanksgiving meal, including mashed potatoes, stuffing, cranberries and more. In fiscal year 2013 - 2014, 1,300 families received thanksgiving assistance from help of southern nevada. Holiday toy assistance allows up to 3,000 needy southern nevada families to receive holiday toys for their eligible children directly through help who, without this assistance, would otherwise do without. In fiscal year 2013 - 2014, 2,599 families received holiday toy assistance from help of southern nevada. Adopt-a-family allows community members the opportunity to 'adopt' local pre-qualified families from the pool of those who apply for holiday assistance through help, who find themselves in unique and compelling situations during the holiday season. Sponsors purchase items from a list of toy and clothing preferences and these items are provided to the pre- qualified family anonymously. In fiscal year 2013 - 2014, 451 families were "adopted" for the holidays through help of southern nevada.

Recently lost his or her primary source of income due to job lay-offs, divorce, death, or disability. In addition, through the workforce investment act-youth program, funded by workforce connections, help of southern nevada provided low income, at-risk youth, in-school and out-of- school youth, ages 16-21 in clark county to include the unincorporated areas of sandy valley and searchlight with long term outcomes such as: basic educational and employment skills, completion of academic certificates (including high school diplomas and equivalents) and placement in employment, post-secondary education and/or training. In 2013/2014, help served 1,420 unduplicated at-risk youth with wia programs. Framing hope warehouse (fhw) provides donated home depot products to registered 501 c 3 organizations for a low administrative fee. Fhw has 82 registered members and in our first year of operations have cumulatively saved member organizations over 500,000.

Community alternative sentencing (cas) program allows court appointed volunteers the opportunity to serve the community, save taxpayers the cost of incarceration, and provide manpower to non-profits and government agencies. During the 2013/2014 fiscal year, community service workers contributed a total of 207,745 hours. State of nevada 211 offers free information and referrals state-wide regarding essential human services, medical and mental health resources, employment support services, programs for children, youth and families, support to seniors and persons with disabilities, volunteer opportunities, and donations and support for community crisis or disaster recovery. Call centers are open 24/7, callers may text their 5 digit zip code to 898211 or visit the website at www.nevada211.org. During the 2013/2014 fiscal year, 63,323 callers were given assistance through this program. Emergency resource services (ers) provides assistance to low-income households and families and individuals that are at imminent risk of homelessness or literally homeless. Services provided include, but are not limited to, case management, rental and utility assistance, food vouchers, clothing vouchers, application for supplemental nutrition assistance program (snap) benefits, application for energy assistance program (eap) or reach, case management and supportive services for individuals with aids/hiv, and assistance with one-way greyhound bus tickets. During fiscal year 2013/2014, ers received 4,924 applications for services. Below are unduplicated numbers for households receiving specific services: "651 interviews for snap benefits and applications submitted "365 aids/hiv clients provided supportive services and case management "59 households provided assistance with reach applications submitted "247 households provided assistance with eap applications "351 households were provided rental assistance, avoiding homelessness "187 households were provided utility assistance, avoiding disconnection of service "4,451 households were provided with food vouchers for pantry services "563 households received gift cards to smith's/albertsons grocery stores "13 individuals were provided assistance with bus fare to be reunited with their families also under ers are our baby first services (bfs) and a new path (anp) program. Bfs provides pre and postnatal guidance to first-time mothers, at-risk teens, and at-risk women in our community to reduce the risk of low birth weight and infant mortality. During the 2013/2014 fiscal year, 471 pregnant or parenting women and their babies were assisted. Some services provided by bfs include, but are not limited to: case management support, assistance with food cards, bus passes, diapers and wipes, formula, play crib, and other baby items. A new path is a tenant based rental assistance program. In fiscal year 2013/2014, 37 unduplicated households were provided with housing for a period ranging from 6 to 12 months. The rent and utility assistance is provided to low-income individuals/families that are homeless or facing homelessness. The program focuses on providing case management, programming and support to its clients to become self-sufficient within 12-months. Shannon west homeless youth center (swhyc) is a residential program offered to at-risk and homeless youth, age 16-24, providing shelter, case management, substance abuse counseling, educational and vocational counseling, and supportive services. The swhyc helped 245 youth in the 2013/2014 fiscal year.

The organization is a not-for-profit organization as described in section 501(c)(3) of the internal revenue code and is generally exempt from income taxes on related income pursuant to the appropriate section of the internal revenue code. In the preparation of tax returns, tax positions are taken based on interpretation of federal, state, and local income tax laws. In accordance with the accounting standards, management periodically reviews and evaluates the status of uncertain tax positions and makes estimates of amounts, including interest and penalties, ultimately due or owed. No amounts have been identified or recorded as uncertain tax positions. IRS990/Desc0HOMELESS SERVICES HELP OF SOUTHERN NEVADA (HELP) HAS BEEN PROVIDING HOUSING AND CASE MANAGEMENT TO HOMELESS INDIVIDUALS/FAMILIES FOR OVER 20 YEARS. HELP HAS BEEN WORKING WITH THE CHRONICALLY HOMELESS SINCE 2005 WHEN THE PILOT PROJECT ORGANIZATIONS UNITED TO REACH EDUCATE AND ASSIST THE CHRONICALLY HOMELESS (O.U.T.R.E.A.C.H.) BEGAN. THIS PROJECT WAS IMPLEMENTED TO ASSIST THE CHRONICALLY HOMELESS USING THE HOUSING FIRST AND HARM REDUCTION MODEL, IN WHICH CLIENTS ARE IMMEDIATELY HOUSED AND THEN BARRIERS ARE WORKED ON. HELP'S NEW BEGINNINGS AND HELP THEM HOME PROGRAMS PROVIDE HOUSING AND INTENSIVE CASE MANAGEMENT TO CHRONICALLY HOMELESS INDIVIDUALS. THE PROGRAM FOCUSES ON PROVIDING INTENSIVE CASE MANAGEMENT, PROGRAMMING AND SUPPORT TO ITS CLIENTS TO BECOME SELF SUFFICIENT ONCE THEY LEAVE THE PROGRAM. HELP'S PATH PROGRAM SERVES THE SEVERELY MENTALLY ILL CLIENTS. PATH HAS BEEN SERVING CLIENTS SINCE MARCH 2013. THESE CLIENTS ARE CONNECTED WITH SERVICE TO STABILIZE THEIR MENTAL HEALTH ISSUES AND ADDRESS OTHER BARRIERS THAT HAVE LEAD TO THEIR HOMELESSNESS. A NEW START PROGRAM (ANS) HAS BEEN PROVIDING HOUSING AND CASE MANAGEMENT TO FAMILIES AND COUPLES SINCE MARCH 2012. ANS IS A SUPPORTIVE HOUSING PROGRAM THAT PROVIDES RENTAL ASSISTANCE AND SUPPORTIVE SERVICES TO HOMELESS FAMILIES/COUPLES WHO HAVE A DOCUMENTED DISABILITY. THE PROGRAM FOCUSES ON PROVIDING CASE MANAGEMENT, PROGRAMMING AND SUPPORT TO ITS CLIENTS TO BECOME SELF SUFFICIENT ONCE THEY LEAVE THE PROGRAM. -NUMBER OF CLIENTS WHO PARTICIPATED IN THE PROGRAM YEAR-NEW BEGINNINGS - 212, HELP THEM HOME -62, OUTREACH - 569, PATH - 366, ANS - 161. -NUMBER OF CLIENTS DISCHARGED THROUGHOUT THE PROGRAM YEAR: NEW BEGINNINGS - 83, HELP THEM HOME - 23, OUTREACH - 540, PATH - 244, ANS - 54. -NUMBER OF CLIENTS BROUGHT ON TO CASELOAD: NEW BEGINNINGS - 212, HELP THEM HOME - 62, OUTREACH -29, PATH - 219, ANS - 161. -NUMBER OF ACTIVE CLIENTS AT THE END OF THE PROGRAM YEAR: NEW BEGINNINGS - 138, HELP THEM HOME - 33, OUTREACH - 29, PATH - 46, ANS - 39. -PERCENTAGE OF CLIENTS WITH SUBSTANCE ABUSE ISSUES: NEW BEGINNINGS - 134 OR 63.2%, HELP THEM HOME - 46 OR 74.2 %, OUTREACH - 218 OR 38.2%, PATH - 242 OR 66.1%, ANS - 97 OR 60.3% -PERCENTAGE OF CLIENTS WITH STABLE EXITS FROM THE PROGRAM: NEW BEGINNINGS - 33 OR 15.5%, HELP THEM HOME - 6 OR .06%, OUTREACH - 29 OR 1 %, PATH - 43, ANS - 6 -PERCENTAGE OF CLIENTS WHO ARE VETERANS: NEW BEGINNINGS - 12OR 5.7%, HELP THEM HOME - 3 OR 4.7%, OUTREACH - 32OR 5.6%, PATH - 37 OR 10.1%, ANS - 1 OR 1%. COMMUNITY OUTREACH HELP HOLIDAY PROGRAMS CONSIST OF THANKSGIVING ASSISTANCE, HOLIDAY TOY ASSISTANCE, AND ADOPT-A-FAMILY. CLIENTS RECEIVING THANKSGIVING ASSISTANCE RECEIVE A FROZEN TURKEY AND GROCERIES TO PREPARE A TRADITIONAL THANKSGIVING MEAL, INCLUDING MASHED POTATOES, STUFFING, CRANBERRIES AND MORE. IN FISCAL YEAR 2013 - 2014, 1,300 FAMILIES RECEIVED THANKSGIVING ASSISTANCE FROM HELP OF SOUTHERN NEVADA. HOLIDAY TOY ASSISTANCE ALLOWS UP TO 3,000 NEEDY SOUTHERN NEVADA FAMILIES TO RECEIVE HOLIDAY TOYS FOR THEIR ELIGIBLE CHILDREN DIRECTLY THROUGH HELP WHO, WITHOUT THIS ASSISTANCE, WOULD OTHERWISE DO WITHOUT. IN FISCAL YEAR 2013 - 2014, 2,599 FAMILIES RECEIVED HOLIDAY TOY ASSISTANCE FROM HELP OF SOUTHERN NEVADA. ADOPT-A-FAMILY ALLOWS COMMUNITY MEMBERS THE OPPORTUNITY TO 'ADOPT' LOCAL PRE-QUALIFIED FAMILIES FROM THE POOL OF THOSE WHO APPLY FOR HOLIDAY ASSISTANCE THROUGH HELP, WHO FIND THEMSELVES IN UNIQUE AND COMPELLING SITUATIONS DURING THE HOLIDAY SEASON. SPONSORS PURCHASE ITEMS FROM A LIST OF TOY AND CLOTHING PREFERENCES AND THESE ITEMS ARE PROVIDED TO THE PRE- QUALIFIED FAMILY ANONYMOUSLY. IN FISCAL YEAR 2013 - 2014, 451 FAMILIES WERE "ADOPTED" FOR THE HOLIDAYS THROUGH HELP OF SOUTHERN NEVADA.
