IN 2012, THE ORGANIZATION PROVIDED DECENT, SAFE, SANITARY AND AFFORDABLE HOUSING TO 31 RESIDENTS: 32% BETWEEN THE AGES 18-61 (NON-ELDERLY PEOPLE WITH DISABILITIES) AND 68% BETWEEN THE AGES OF 62-85. MANY RESIDENTS HAD DIRECT SERVICES IN PLACE AND CASE MANAGEMENT THROUGH THE PIMA COUNCIL ON AGING. THESE CASE MANAGERS PROVIDED ONE-ON-ONE ASSESSMENTS AND COORDINATED SERVICES FOR THEIR PERSONAL CARE, HOME HEALTH, SUPPORTIVE COUNSELING, MEALS ON WHEELS AND FOOD BOXES. SOME RESIDENTS HAD CASE MANAGEMENT THROUGH C.O.P.E. BEHAVIORAL HEALTH FOR ADDITIONAL MENTAL HEALTH SERVICES. (CONTINUED ON SCH O) THE NEARBY UDALL PARK SENIOR CENTER WAS AVAILABLE FOR RESIDENTS TO PARTICIPATE IN ADDITIONAL PROGRAMS AND ACTIVITIES. THE ON-SITE MANAGER WORKED WITH PIMA COUNTY HEALTH DEPARTMENT TO BRING PROGRAMS ON-SITE TO ENGAGE THE RESIDENTS IN A VARIETY OF HEALTH AND WELLNESS ACTIVITIES, INCLUDING SMOKING CESSATION CLASSES. RESIDENTS PARTICIPATED IN AN IN-SERVICE FOR MEDICARE UPDATES AND ISSUES. THE MANAGER ALSO PLANNED BIRTHDAY PARTIES, BARBECUES, ICE CREAM SOCIALS, GARDENING ACTIVITIES AND OTHER SOCIAL ACTIVITIES TO KEEP THE RESIDENTS ACTIVE AND INVOLVED IN THE COMMUNITY.
