Activity 2

Food and Nutrition Program provides balanced nutrition for children enrolled at Phoenix Day, quarterly nutritional information and congregate meals for the community, and emergency food box referrals to families experiencing crisis situations. All children receive two meals and two to three snacks daily. The menus and portion sizes are determined in accordance with the Child and Adult Care Food Program to ensure the utmost quality in healthy and nutritious food. The program teaches good eating habits in youth to help children lead healthier lives as adults.

Activity 3

Health Links Program offers assistance in accessing free or low cost health care and insurance programs and coordination of preventative health services including vision, hearing, height/weight screenings, dental varnish applications and developmental assessments. The program provides community information, education, resources and referrals to community based services as needed. Outreach programs include weekly and monthly recurring on-site presence of partner agencies, health fairs and community events, scheduled appointments at the facility and agency referrals. The program reaches 876 individuals through outreach events, community sites, community events and health fairs. Health Links provides 25 Emergency Box vouchers and referrals 400 people attend Nutrition Nights 187 families are assisted through Waste Not and 274 families are assisted through other programs and assistance.
