Form 990, Part III, Line 1 - Organizational Mission

When jeremy, who experiences down syndrome, was looking to move into his own apartment, the only available options were restrictive, one-size-fits-all group homes and supervised-apartments focused on safety rather than growth and often not in walkable locations, or generic market apartments that lacked affordability and a sense of community. Inspired by jeremy's needs and aspirations, his brother nathaniel sought to create a vibrant and inclusive housing program. In 2017, nathaniel and the diskint family founded cohome, a 501(c)3 not-for-profit organization dedicated to advocating for inclusion through an innovative coliving model. Cohome's person-centered approach nurtures self-advocacy, independence, and meaningful connections, ensuring that all individuals can thrive in a supportive and inclusive environment. In 2023, the new jersey supportive housing association awarded cohome their prestigious innovation award, recognizing cohome's innovative approach to housing and programs for adults with disabilities and cohome's leadership advocating for disability-inclusion in new jersey.

Form 990, Part III, Line 4A - Program Accomplishments

In 2023, cohome's unique disability-inclusive coliving program impacted over 150 individuals through meaningful shared-experiences. Cohome's flagship home continued to provide disability-inclusive housing and residential programing to 9 neurodiverse individuals. Cohome's volunteer program saw a huge increase in interest and participation by over 100 local high school students. To address the need for meaningful social experiences in late-afternoon and weekends, cohome also successfully launched a community-facing social skills pilot program serving 12 individuals. 2023 also saw the completion of phase ii renovations on our flagship home, including interior renovations and development approvals for an accessory residential structure on the property to be developed in 2024. Cohome also focused on developing organizational capacity to continue expanding the housing model through 2023 into 2024.

IN 2017, NATHANIEL AND THE DISKINT FAMILY FOUNDED COHOME, A 501(C)3 NOT-FOR-PROFIT ORGANIZATION DEDICATED TO ADVOCATING FOR INCLUSION THROUGH AN INNOVATIVE COLIVING MODEL. COHOME'S PERSON-CENTERED APPROACH NURTURES SELF-ADVOCACY, INDEPENDENCE, AND MEANINGFUL CONNECTIONS, ENSURING THAT ALL INDIVIDUALS CAN THRIVE IN A SUPPORTIVE AND INCLUSIVE ENVIRONMENT. IN 2023, THE NEW JERSEY SUPPORTIVE HOUSING ASSOCIATION AWARDED COHOME THEIR PRESTIGIOUS INNOVATION AWARD, RECOGNIZING COHOME'S INNOVATIVE APPROACH TO HOUSING AND PROGRAMS FOR ADULTS WITH DISABILITIES AND COHOME'S LEADERSHIP ADVOCATING FOR DISABILITY-INCLUSION IN NEW JERSEY.

TO ADDRESS THE NEED FOR MEANINGFUL SOCIAL EXPERIENCES IN LATE-AFTERNOON AND WEEKENDS, COHOME ALSO SUCCESSFULLY LAUNCHED A COMMUNITY-FACING SOCIAL SKILLS PILOT PROGRAM SERVING 12 INDIVIDUALS. 2023 ALSO SAW THE COMPLETION OF PHASE II RENOVATIONS ON OUR FLAGSHIP HOME, INCLUDING INTERIOR RENOVATIONS AND DEVELOPMENT APPROVALS FOR AN ACCESSORY RESIDENTIAL STRUCTURE ON THE PROPERTY TO BE DEVELOPED IN 2024. COHOME ALSO FOCUSED ON DEVELOPING ORGANIZATIONAL CAPACITY TO CONTINUE EXPANDING THE HOUSING MODEL THROUGH 2023 INTO 2024.
