Our mission is to strengthen the capacity of uninsured, low-income individuals and their families to thrive by providing free healthcare and health supporting services, meeting their immediate needs, and serving as a training site for students to explore careers in healthcare.

Form 990, Part III, Line 2

1. Llegando a la meta weight loss program: this is a train-the-trainer program where we teach patients with weight issues how to help others in their family or community lose weight and/or eat health to prevent weight gain, while they practice on themselves. 2. Salud (student advocates leading uplifting decisions): salud is a word that means health in spanish. The group (currently 29 bilingual high school clinic interns whose families are patients of the clinic) was formed to benefit the majority of our patient population and empower the youth of the latino community to be leaders and advocates for health. In the process the students are changing the food options in the ventura county communities they come from and driving business to the latino markets willing to offer heart healthy, diabetic friendly foods to the organization's patients and the general community. The students have created a business model that benefits everyone and promotes health for everyone. Leadership training, budgeting and program design and implementation are all part of the student experience as they come up with new ideas of how to advocate for their community's health. 3. Corazones sanos (healthy hearts) program: this program assists adults and children at risk for heart disease, including those who have type 2 diabetes, with education and early treatment. This program also helps prevent children from getting heart disease/diabetes like their parents. High school interns and a college student work in partnership with the bilingual program coordinator, health educator and dietitian to implement the program. Program participants are encouraged to get involved too. While they are helping they are also learning how they can improve their own family's health. The comprehensive program includes: 1) personal dietary and exercise planning, goal setting and monitoring, 2) quarterly lab tests, 3) weekly glucose finger sticks, 4) weight and circumference measuring, 5) weekly nutrition classes and food demonstrations, 6) visits with the dietician, 7) free medications, glucometer and test strips when needed, 8) appointments with our podiatrist and ophthalmologist, 9) free fruits and vegetables, as well as zumba and yoga exercise classes. 4. IT IS THE GOAL OF THE ORGANIZATION TO PROVIDE: 1) EASY ACCESS TO FREE MEDICAL CARE TO HARD-TO-REACH POPULATIONS FACING TRANSPORTATION, LANGUAGE, CULTURAL AND FINANCIAL BARRIERS TO CARE; AND 2) PROVIDE HIGH SCHOOL STUDENTS WITH A PATHWAY TO CAREERS IN HEALTH THROUGH EXPOSURE AND HANDS-ON TRAINING IN THE DELIVERY OF HEALTH CARE. THE ORGANIZATION IS A NONPROFIT HEALTH AND HUMAN SERVICES ORGANIZATION EXEMPT FROM FEDERAL INCOME TAXES UNDER SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE (CODE). LLEGANDO A LA META WEIGHT LOSS PROGRAM: THIS IS A TRAIN-THE-TRAINER PROGRAM WHERE WE TEACH PATIENTS WITH WEIGHT ISSUES HOW TO HELP OTHERS IN THEIR FAMILY OR COMMUNITY LOSE WEIGHT AND/OR EAT HEALTH TO PREVENT WEIGHT GAIN, WHILE THEY PRACTICE ON THEMSELVES. 2. SALUD (STUDENT ADVOCATES LEADING UPLIFTING DECISIONS): SALUD IS A WORD THAT MEANS HEALTH IN SPANISH. THE GROUP (CURRENTLY 29 BILINGUAL HIGH SCHOOL CLINIC INTERNS WHOSE FAMILIES ARE PATIENTS OF THE CLINIC) WAS FORMED TO BENEFIT THE MAJORITY OF OUR PATIENT POPULATION AND EMPOWER THE YOUTH OF THE LATINO COMMUNITY TO BE LEADERS AND ADVOCATES FOR HEALTH. IN THE PROCESS THE STUDENTS ARE CHANGING THE FOOD OPTIONS IN THE VENTURA COUNTY COMMUNITIES THEY COME FROM AND DRIVING BUSINESS TO THE LATINO MARKETS WILLING TO OFFER HEART HEALTHY, DIABETIC FRIENDLY FOODS TO THE ORGANIZATION'S PATIENTS AND THE GENERAL COMMUNITY. THE STUDENTS HAVE CREATED A BUSINESS MODEL THAT BENEFITS EVERYONE AND PROMOTES HEALTH FOR EVERYONE. LEADERSHIP TRAINING, BUDGETING AND PROGRAM DESIGN AND IMPLEMENTATION ARE ALL PART OF THE STUDENT EXPERIENCE AS THEY COME UP WITH NEW IDEAS OF HOW TO ADVOCATE FOR THEIR COMMUNITY'S HEALTH. 3. CORAZONES SANOS (HEALTHY HEARTS) PROGRAM: THIS PROGRAM ASSISTS ADULTS AND CHILDREN AT RISK FOR HEART DISEASE, INCLUDING THOSE WHO HAVE TYPE 2 DIABETES, WITH EDUCATION AND EARLY TREATMENT. THIS PROGRAM ALSO HELPS PREVENT CHILDREN FROM GETTING HEART DISEASE/DIABETES LIKE THEIR PARENTS. HIGH SCHOOL INTERNS AND A COLLEGE STUDENT WORK IN PARTNERSHIP WITH THE BILINGUAL PROGRAM COORDINATOR, HEALTH EDUCATOR AND DIETITIAN TO IMPLEMENT THE PROGRAM. PROGRAM PARTICIPANTS ARE ENCOURAGED TO GET INVOLVED TOO. WHILE THEY ARE HELPING THEY ARE ALSO LEARNING HOW THEY CAN IMPROVE THEIR OWN FAMILY'S HEALTH. THE COMPREHENSIVE PROGRAM INCLUDES: 1) PERSONAL DIETARY AND EXERCISE PLANNING, GOAL SETTING AND MONITORING, 2) QUARTERLY LAB TESTS, 3) WEEKLY GLUCOSE FINGER STICKS, 4) WEIGHT AND CIRCUMFERENCE MEASURING, 5) WEEKLY NUTRITION CLASSES AND FOOD DEMONSTRATIONS, 6) VISITS WITH THE DIETICIAN, 7) FREE MEDICATIONS, GLUCOMETER AND TEST STRIPS WHEN NEEDED, 8) APPOINTMENTS WITH OUR PODIATRIST AND OPHTHALMOLOGIST, 9) FREE FRUITS AND VEGETABLES, AS WELL AS ZUMBA AND YOGA EXERCISE CLASSES. 4.
