Mtppi's board of directors performs an annual evaluation and determination of the salary cap. This procedure is done through comparative analysis of similar organizations in order to determine appropriate salary and/or bonus adjustments. Conversely, the president of mtppi performs an annual evaluation of employees and compares their salaries to similar organizations in order to determine appropriate salary/or bonus adjustments. In addition, mtppi employs a merit method of evaluating salaries of the employees.

THIS RESEARCH PROJECT EXAMINED WHETHER THE BLACK/WHITE DISPARITY IN AVF USE IS AFFECTED BY SURGEON SUPPLY. USING CROWNWEB AND MEDICARE CLAIMS DATA FROM THE U.S. RENAL DATA SYSTEM, COMPETING RISK ANALYSES OF ALL U.S. PATIENTS INITIATING HEMODIALYSIS WITH A CENTRAL VENOUS CATHETER (CVC) FROM 2016-2017 (N=103,286) WERE PERFORMED. THE LIKELIHOOD OF SUCCESSFUL AVF USE WAS COMPARED BETWEEN BLACK AND WHITE PATIENTS AFTER ADJUSTING FOR VASCULAR ACCESS SURGEON SUPPLY, CALCULATED AS THE NUMBER OF SURGEONS AVAILABLE PER 1,000 HEMODIALYSIS PATIENTS IN EACH HOSPITAL REFERRAL REGION. THE LOWER RATE OF AVF USE IN BLACK, AS COMPARED TO WHITE, HD PATIENTS HAS BEEN REPEATEDLY DEMONSTRATED WITH BLACK PATIENTS HAVING AN ABSOLUTE 10% LOWER RATE OF AVF USE. HOWEVER, THE EXPLANATION FOR THIS RACIAL DISPARITY IN AVF USE REMAINS POORLY UNDERSTOOD. ALTHOUGH, ON AVERAGE, BLACK AND WHITE HEMODIALYSIS PATIENTS HAVE DIFFERENT DEMOGRAPHIC PROFILES, CO-MORBID DISEASE BURDEN, ACCESS TO MEDICAL CARE, INCOME, AND MEDICAL INSURANCE, THE LOWER AVF USE AMONG BLACK PATIENTS PERSISTS EVEN AFTER ROBUST STATISTICAL ADJUSTMENT FOR THESE FACTORS. RESEARCHERS RECENTLY REPORTED SIGNIFICANT DIFFERENCES IN SEVERAL AVF PROCESSES OF CARE BETWEEN OLDER ADULT BLACK AND WHITE HD PATIENTS. BLACK PATIENTS STARTING HD WITH A CVC WERE LESS LIKELY TO HAVE AN AVF PLACED, LESS LIKELY TO HAVE IT MATURE FOLLOWING PLACEMENT, AND LESS LIKELY TO MAINTAIN AVF PATENCY AFTER ITS MATURATION

BY DESIGN, ESRD NETWORK 5 HAS DEMONSTRATED EFFECTIVENESS IN RAPIDLY TRANSLATING NEW RESEARCH FINDINGS INTO CLINICAL PRACTICE. RESEARCHERS DIRECTLY INTEGRATED PCORI STUDY FINDINGS INTO NETWORK 5'S QUALITY IMPROVEMENT WORK WHICH IS BELOW IN THIS REPORT. SPECIFICALLY, PROJECT DISSEMINATION EFFORTS ADDRESSED THE TASKS OF HELPING NETWORK 5 PROVIDE PEER MENTORING FRAMEWORKS AND INFRASTRUCTURE TO STAKEHOLDERS, HELP PATIENTS WITH DEPRESSION, HELP PATIENTS AND CAREGIVERS NAVIGATE THE MODALITY SELECTION PROCESS, AND PROVIDE SUPPORTIVE END OF LIFE CARE. RESEARCHERS LEVERAGED THE EXISTING D&I NETWORK 5 INFRASTRUCTURE AND ACCESS TO 400+ DIALYSIS FACILITIES, TO ENGAGE PATIENTS, AND TO PROVIDE INFORMATION AND TOOLS FOR FUTURE TRANSLATION OR PCORI-FUNDED EVIDENCE INTO ROUTINE ESRD CARE THROUGHOUT NETWORK 5 AND VIA THE NETWORK'S ROUTINE DISSEMINATION OF SUCCESSFUL PROGRAMS TO THE OTHER 17 NETWORKS.

IRS990/ProgSrvcAccomActy3Grp/Desc0MTPPI USED THE MOST RECENT MEDICARE CLAIMS DATA (2017-JUNE 2020 OBTAINED VIA RESDAC) AND 2016 UNITED STATES RENAL SYSTEM DATA (USRDS) TO CONDUCT THIS CROSS-SECTIONAL STUDY TO ASSESS ASSOCIATIONS BETWEEN ADOPTION OF TRIFERIC AND HEMOGLOBIN LEVELS, ESA AND IV-IRON REQUIREMENTS, AND PATIENT OUTCOMES INCLUDING HOSPITALIZATION AND MORTALITY. RESEARCHERS HYPOTHESIZED THAT 1) PATIENTS TREATED IN FACILITIES THAT ADOPTED TRIFERIC WILL MAINTAIN HEMOGLOBIN LEVELS DESPITE LESS UTILIZATION OF IV IRON AND ESAS AND WITHOUT CAUSING TISSUE IRON OVERLOAD AS SUGGESTED BY NO INCREASE IN SERUM FERRITIN VALUES, AND 2) COMPARED TO PRE-TRIFERIC PERIOD WITH IV-IRON ONLY, IMPLEMENTATION OF TRIFERIC IS ASSOCIATED WITH DECREASED ALL-CAUSE, INFECTION AND CARDIOVASCULAR HOSPITALIZATION AND ALL-CAUSE MORTALITY. ALL ADULT (>=18) MEDICARE PATIENTS UNDERGOING IN-CENTER HEMODIALYSIS BETWEEN 2016 AND 2020 WERE INCLUDED IN THE STUDY. OUTPATIENT DIALYSIS CLAIMS WERE IDENTIFIED AS ALL CLAIMS FOR DIALYSIS PATIENTS WITH THE PROVIDER TYPE CODE FOR DIALYSIS FACILITIES (CODE 72: CLINICHOSPITAL BASED OR INDEPENDENT RENAL DIALYSIS FACILITY). IDENTIFYING DIALYSIS CLAIMS VIA PROVIDER TYPE, RATHER THAN THE PRESENCE ON THE CLAIM OF AT LEAST ONE ITEM WITH A DIALYSIS REVENUE CENTER, YIELDS A MORE COMPLETE SET OF DIALYSIS-RELATED CLAIMS. PATIENTS RECEIVING IN-CENTER HEMODIALYSIS ARE IDENTIFIED BY A "72X" TYPE OF BILL (WHERE X IS ANY VALUE), AND A "0821" REVENUE CENTER CODE FOR EACH TREATMENT OCCURRENCE. THERE WAS A TOTAL OF 1,996 PATIENTS TREATED IN 8 TRIFERIC FACILITIES BETWEEN JAN 1, 2016 AND JUNE 30, 2020; 1,113 PATIENTS IN PRE-TRIFERIC PERIOD AND 883 IN POST-TRIFERIC PERIOD. THE TOTAL NUMBER OF DIALYSIS TREATMENT MONTHS WAS 21,568. ALL 21,568 DIALYSIS TREATMENT MONTHS WERE INCLUDED IN THE ANALYSIS REGARDLESS WHETHER AN ESA OR IV-IRON TREATMENT WAS ADMINISTERED IN THE MONTHS. AS EXPECTED, A 65% DECREASE IN THE NUMBER OF IV-IRON TREATMENT MONTHS WAS OBSERVED FROM PRE-TRIFERIC PERIOD (50.4%) TO POST-TRIFIRIC (17.4%). IN CONTRAST, USE OF ESA (74% PRE VS 77% POST-TRIFERIC) AND HOSPITALIZATION RATE (15% PRE VS. 14% POST-TRIFERIC) WERE FOUND TO BE SIMILAR.
