| IRS990/Desc | 0 | In 2019, UCare Foundation approved and disbursed $1,750,394 in grant funding to twenty-one Minnesota organizations working to improve the overall health of people of all ages, cultures, and abilities through innovative services, education, community outreach and research. UCare Foundation grants were awarded to organizations meeting one of four health improvement-related criteria for 2019:- Behavioral Health Services- Social Connectedness (Isolation)- Social Determinants of Health- Promoting Health EquityIn 2019, grants provided by the Foundation using funds contributed by UCare included support for three groups: clinical grants, community grants and county grants. Initiatives within those groups included: funding to provide companionship for seniors; a health and wellbeing program for Somali women; a diabetes prevention program for urban-dwelling American Indians and several organizations beginning to offer Medication Assisted Treatment options. UCare Foundation has a formal board that oversees and approves annual grant awards. The board meets annually to determine priority areas of focus for current year solicitations. Grantees apply via an online grant system and all organizations must meet funding eligibility guidelines. Grant applications are reviewed by UCare Minnesota-assigned individuals as well as certain members of the UCare Foundation board. Once approved, grantees sign a conditions of grant agreement and are responsible to submit an interim and final report as well as participate in a site visit by UCare staff. The Board of Directors receives an annual report on the use of the funds.During the year, UCare staff monitored the activities and received reports related to a number of grants in progress from 2018 including support to organizations that: promote wellness and personal support activities using the social determinants of health model, offer social supports and transportation to seniors living at home, develop online resources and tools to support culturally appropriate delivery of health care, and provide programs to reduce health and opportunity disparities for North Minneapolis youth by getting them outdoors and active, and setting them on a path for long-term health and success. |
