| IRS990/Desc | 0 | In 2017, UCare Foundation distributed 3 community grants to Minnesota organizations working to improve the health of people of all ages, cultures, and abilities through innovative services, education, community outreach and research. UCare Foundation grants were awarded to organizations meeting one of three health improvement-related criteria for 2017:-Preventive health care-Programs for people with disabilities-Promoting health equityExamples of UCare Foundation grants awarded in 2017 include support for organizations that promote education and research related to primary care and family medicine, activities that support health disparities and healthy living among southern Minnesota refugee families, and programs that provide various supports to aging seniors who are struggling to live independently in the community. UCare Foundation has a formal board that oversees and approves annual grant awards. The board meets annually to determine priority areas of focus for current year solicitations. Grantees apply via an online grant system and all organizations must meet funding eligibility guidelines. Grant applications are reviewed by UCare Minnesota assigned individuals as well as certain members of the UCare Foundation board. Once approved grantees sign a conditions of grant agreement and are responsible to submit an interim and final report as well as participate in a site visit by UCare staff. The Board of Directors receives an annual report on the use of the funds.During the year, UCare staff monitored the activities and received reports related to a number of grants in progress from 2016, including support to organizations that promote access, adventure, and opportunities for those with disabilities, assist refugee families to secure basic needs and resources for self-sufficiency and to acclimate to a new way of life, expansion of the Twin Cities Mobile Market that increased access to healthy, affordable food for low income residents, provide legal supports for seniors, provide online resources and tools to support culturally appropriate delivery of health care, deliver child advocacy programs that address issues related to poverty, abuse or neglect, health care access, and quality education, and provide programs to reduce health and opportunity disparities for North Minneapolis youth by getting them outdoors and active, and setting them on a path for long-term health and success. |
