“Ms. Uyen Khue Le has been the organization's part-time CFO-Secretary and she is the half-sister of Mr. Phuong Nhan Le, who is the CEO of the organization. Ms. Uyen Khue Le has transitioned to be on a consultant basis in 2015 to prepare the organization to hire a Finance/Secretary staff. This temporary arrangement will continue until the organization can find a more permanent replacement.”

“The 990 Form will be completed by Dr. Phuong N. Le, the CEO and Director, and will be sent to the three other directors (Dr. Mary McDonnell, Dr. Hub Langstaff, and Mr. Tony Proscio) and the organization's CFO/Secretary, Ms. Uyen Khue Le, for their comments and inputs prior to the formal filing to the IRS.”

“The potential for conflict of interest is brought up as part of every Board of Directors' meeting. The grantees and potential grantees are reviewed in terms of possible conflict of interest with each director as well with any staff. Annual statement disclosing possible or on-going conflict of interest is signed by each director and staff of the organization and maintained on file by the organization. The organization's conflict of interest policy was last revised by a sub-committee in 2016.”

“The salary for the CEO position was determined in December 2013 by looking at the best practice level of the industry and the size of the organization, balancing it with the necessity of living expenses part-time in Viet Nam and in the US. The report was presented to the Board of Directors for their approval (Dr. Phuong Le abstained from voting on this). The decision was made at the level of $72,000 per year for a full-time CEO position. Likewise, a similar process was carried out for the Secretary-Treasurer position and the decision was made at the $17,500 per year level, which reflects the part-time nature of the workload.”

“The organization just recently established its website at www.resourcesforhealthequity.org and has already included its financial statements and completed 990 forms for 2013 to the present. It is in the process of putting all of its governing documents as well in this website. Also, all these documents can be requested by mail or telephone to the organization's address.”

“The organization received a grant from the CMB Foundation to implement The Equity Initiative, a fellowship program to identify and nurture young leaders in the Southeast Asia region to improve health equity. The program is based, in part, out of RHE's Viet Nam Country Office.”

“The organization successfully completed its contractual obligations to The Atlantic Philanthropies on the management of the Population Health Program portfolio of projects at the end of 2016.”
