“An independent accounting firm prepares and reviews the 990. The 990 is then reviewed by the organization's accounting personnel. Any questions or concerns the organization's accounting personnel have are addressed and any corrections that need to be made are made. The 990 is then reviewed by the organization's executive director and any corrections or clarifications that need to be made are made. The final form 990 with all required schedules is then provided to all voting members of the board prior to filing the 990.”

“The conflict of interest policy for the family and community trust (fact) requires that all staff members, board members and volunteers disclose annually and in writing to the board of directors all conflicts of interest. Following full disclosure of a possible conflict of interest or any condition listed above, the board of directors shall determine whether a conflict of interest exists and, if so the board shall vote to authorize or reject the transaction or take any other action deemed necessary to address the conflict and protect facts best interests. Both votes shall be by a majority vote without counting the vote of any interested director, even if the disinterested directors are less than a quorum provided that at least one consenting director is disinterested. An interested board member, officer, or staff member sall not participate in any discussion or debate of the board of directors, or of any committee or subcommittee thereof in which the subject of discussion is a contract, transaction, or situation in which there may be a perceived or actual conflict of interest.”

“FACT GENERAL OPERATING: FACT continues to support the Apricot system and provides funding to support the licensing of users at all 20 Community Partnerships across Missouri. This system is used for budgeting, invoicing, expenditures and tracking. In 2016 the Community Partnerships leveraged on average 8.46 for every dollar of state funding they received. They generated 222,307 hours of volunteer service in their communities of influence. FACT continues to be a major support and funder for the annual ReEntry Conference. This conference each fall attracts nearly 400 participants interested in reentry efforts. The FACT Executive Director gives one of the key opening addresses and FACT staff assist with the logistics of this three day conference. FACT, through four of its Community Partnerships, continue to support the Summer Food Program (SEBTC) in conjunction with the Department of Social Services and the US Department of Agriculture. The FACT Executive Director assisted in the writing of the initial grant and continues to support the work as a member of the team guiding this work in Missouri. FACT continues to support the work of the local chapter of Zonta by supporting their annual Women of Achievement Award. FACT is finishing up its work with Walmart and First Book to provide high quality books to four of its Community Partnerships in the Bootheel of Missouri. This effort is called Books for the Bootheel and will infuse these communities with another 40,000 books for children and youth, ages 0-18. FACT continues to support communities across the state to establish Wonderland Libraries for children. These free libraries are established with funding from the Fraternal Order of Eagles. FACT has been established as the Missouri No Kid Hungry Organization in cooperation with the Share Our Strength (SOS) organization in Washington, DC. This commitment is to eradicate childhood hunger in our state in three years based upon the benchmarks and guidelines established SOS. In 2016 as part of the Missouri Breakfast Challenge, No Kid Hungry Missouri supported 19 schools with grants totaling $49,078.18 to increase participation in and availability of school breakfast. FACT is planning two major conferences to support this effort in 2017 to support this work and will also focus on Summer Food Programs and Afterschool Meal Programs.”

“Kids count: fact is in its third year as the kids count grantee for missouri. This annie e. Casey program is funded with a grant to support this report card on child wellbeing. In 2016 missouri kids count (mkc) accomplished the following things: release of the missouri kids count databook, during child advocacy day. The release events included a kick off with media, partnerships and school children. Presentations by fact board, oseda. Presentation of the new data indicators being considered for the 2017 mkc data release. Mkc participated in/supported the aecf release of the report - "a shared sentence the devastating toll of parental incarceration on kids, families and communities." mkc released its own two-part series focusing on missouri children and parents dealing with incarceration, and the doc programs addressing the needs of these families. Two staff from the mkc team attended the race/equity institute sponsored by aecf. Mkc released the school nurse series throughout late 2015 and during the first two months of 2016. This 4-part series focused on the work of school nurses in missouri, including the first ever publication of annual data collected by school nurses categorizing the diseases and conditions, chronic and acute, of missouri school children. The series received attention from the cdc, from the national associations for school nurses and school health professionals, and some media. Mkc released a report on teen deaths in missouri, after hearing from the partnerships that teen suicide and suicide attempts were noticeably increasing in their communities. Mkc prepared a report for dss on the state annual child death report. Mkc began work on the childrens services tax feature article. Mkc participated in the missouri prevention partners, on the steering committee of moclc, and on the planning committee for cad.”

“STRENGTHENING FAMILY PARTNERSHIPS: FACT Executive Director co-authored a grant proposal to Annie E. Casey Foundation and continues to reapply to continue this work with the Mo Dept. of Social Services, Div. of Youth Services (DYS). FACT continues to be funded to support this important work with DYS using the Full Frame Initiative as the agent of change.”
