“The grants and projects we've been funded by and implemented have progressively built on our relationships with community and agency partners. The upward trajectory of engaged community leadership began with healthy savannah and the ymca of coastal georgia partnering on a 'pioneering healthy communities' grant (2008); then on a 'healthcare georgia foundation, childhood obesity prevention' grant (2013-2017); and a "play streets" grant from the 'partnership for a healthier america', (2013). In culmination, the ymca & healthy savannah received a monumental five-year 'racial and ethnic approaches to community health' grant from the cdc, in september 2018. We follow work strategies along three pillars: nutrition, physical activity, and community & clinical linkages. This grant is proof of a strategically built momentum, of community trust in our work, and our effective community stakeholder and coalition engagement. 2019 healthy savannah coalition accomplishments include: passage of savannah's new zoning ordinance (newzo). The ordinance revises the outdated zoning code and map from 1960. Recently, attitudes toward city planning have shifted and now encourage more bike-ped accessible roadways, paths, and spaces. Healthy savannah was a critical voice advocating for major revisions to the existing ordinance and promoting re-integration of previously shuttered neighborhoods like west savannah. Now with its passage we will continue planning and advocating for development and advancements for these critically ignored neighborhoods. Tide to town - an urban trail system. Healthy savannah has envisioned a safe, pedestrian and bicycle friendly trail system since our founding. In the last year we have successfully seen the 30-mile protected network of activity-friendly trails begin development. The construction of the truman linear park trail and tide to town are victories for mobility and neighborhood integration to everyday destinations. For the next several years, we will focus on mastering the non-displacement of residents and building wealth in the existing community. A significant portion of the trail system intersects with low-wealth neighborhoods that lack mobility options. Engagement and liaising with city and county officials is crucial to making residents feel empowered about the project and proud to remain living beside the trail system. Hero database: healthy savannah and the st. Joseph's candler african american health information & resource center created a health resources database called hero: health effective resource organizations. It compiles over 1000 free or low-cost services in our community on one webpage, searchable by zip code, to connect people to services they didn't know about. The work prepared for a january 2020 launch. (www.herohelpme.com). We continue to prioritize healthy food access high in our vision of a healthier savannah. As such, the successful expansion of farm truck 912's stops and services to low-wealth neighborhoods with low mobility has been a goal of ours. Farm truck 912, the mobile version of the forsyth farmers market, and one of our key nutrition strategy stakeholders, provides and improves access to fresh produce across the community. The truck halves its prices for snap and ebt customers. Healthy savannah has also provided technical assistance and support to the forsyth farmers market in securing a grant for their 'food farmacy', a 475,000 grant over three years. Healthy savannah's work in the last two years has been some of the most focused and successful at moving people out of poverty in our thirteen years. That is the power of our 200-member coalition, our close relationships with the local government leaders, the faith-based community, and residents in our priority population. We will continue to advocate for and implement policy, systems, and environmental change (pse) to achieve a culture of health in the savannah-chatham community.”

THE UPWARD TRAJECTORY OF ENGAGED COMMUNITY LEADERSHIP BEGAN WITH HEALTHY SAVANNAH AND THE YMCA OF COASTAL GEORGIA PARTNERING ON A 'PIONEERING HEALTHY COMMUNITIES' GRANT (2008); THEN ON A 'HEALTHCARE GEORGIA FOUNDATION, CHILDHOOD OBESITY PREVENTION' GRANT (2013-2017); AND A "PLAY STREETS" GRANT FROM THE 'PARTNERSHIP FOR A HEALTHIER AMERICA', (2013). IN CULMINATION, THE YMCA & HEALTHY SAVANNAH RECEIVED A MONUMENTAL FIVE-YEAR 'RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH' GRANT FROM THE CDC, IN SEPTEMBER 2018. WE FOLLOW WORK STRATEGIES ALONG THREE PILLARS: NUTRITION, PHYSICAL ACTIVITY, AND COMMUNITY & CLINICAL LINKAGES. THIS GRANT IS PROOF OF A STRATEGICALLY BUILT MOMENTUM, OF COMMUNITY TRUST IN OUR WORK, AND OUR EFFECTIVE COMMUNITY STAKEHOLDER AND COALITION ENGAGEMENT. 2019 HEALTHY SAVANNAH COALITION ACCOMPLISHMENTS INCLUDE: PASSAGE OF SAVANNAH'S NEW ZONING ORDINANCE (NEWZO). THE ORDINANCE REVISES THE OUTDATED ZONING CODE AND MAP FROM 1960. RECENTLY, ATTITUDES TOWARD CITY PLANNING HAVE SHIFTED AND NOW ENCOURAGE MORE BIKE-PED ACCESSIBLE ROADWAYS, PATHS, AND SPACES. HEALTHY SAVANNAH WAS A CRITICAL VOICE ADVOCATING FOR MAJOR REVISIONS TO THE EXISTING ORDINANCE AND PROMOTING RE-INTEGRATION OF PREVIOUSLY SHUTTERED NEIGHBORHOODS LIKE WEST SAVANNAH. NOW WITH ITS PASSAGE WE WILL CONTINUE PLANNING AND ADVOCATING FOR DEVELOPMENT AND ADVANCEMENTS FOR THESE CRITICALLY IGNORED NEIGHBORHOODS. TIDE TO TOWN - AN URBAN TRAIL SYSTEM. HEALTHY SAVANNAH HAS ENVISIONED A SAFE, PEDESTRIAN AND BICYCLE FRIENDLY TRAIL SYSTEM SINCE OUR FOUNDING. IN THE LAST YEAR WE HAVE SUCCESSFULLY SEEN THE 30-MILE PROTECTED NETWORK OF ACTIVITY-FRIENDLY TRAILS BEGIN DEVELOPMENT. THE CONSTRUCTION OF THE TRUMAN LINEAR PARK TRAIL AND TIDE TO TOWN ARE VICTORIES FOR MOBILITY AND NEIGHBORHOOD INTEGRATION TO EVERYDAY DESTINATIONS. FOR THE NEXT SEVERAL YEARS, WE WILL FOCUS ON MASTERING THE NON-DISPLACEMENT OF RESIDENTS AND BUILDING WEALTH IN THE EXISTING COMMUNITY. A SIGNIFICANT PORTION OF THE TRAIL SYSTEM INTERSECTS WITH LOW-WEALTH NEIGHBORHOODS THAT LACK MOBILITY OPTIONS. ENGAGEMENT AND LIAISING WITH CITY AND COUNTY OFFICIALS IS CRUCIAL TO MAKING RESIDENTS FEEL EMPOWERED ABOUT THE PROJECT AND PROUD TO REMAIN LIVING BESIDE THE TRAIL SYSTEM. HERO DATABASE: HEALTHY SAVANNAH AND THE ST. JOSEPH'S CANDLER AFRICAN AMERICAN HEALTH INFORMATION & RESOURCE CENTER CREATED A HEALTH RESOURCES DATABASE CALLED HERO: HEALTH EFFECTIVE RESOURCE ORGANIZATIONS. IT COMPILES OVER 1000 FREE OR LOW-COST SERVICES IN OUR COMMUNITY ON ONE WEBPAGE, SEARCHABLE BY ZIP CODE, TO CONNECT PEOPLE TO SERVICES THEY DIDN'T KNOW ABOUT. THE WORK PREPARED FOR A JANUARY 2020 LAUNCH. (WWW.HEROHELPME.COM). WE CONTINUE TO PRIORITIZE HEALTHY FOOD ACCESS HIGH IN OUR VISION OF A HEALTHIER SAVANNAH. AS SUCH, THE SUCCESSFUL EXPANSION OF FARM TRUCK 912'S STOPS AND SERVICES TO LOW-WEALTH NEIGHBORHOODS WITH LOW MOBILITY HAS BEEN A GOAL OF OURS. FARM TRUCK 912, THE MOBILE VERSION OF THE FORSYTH FARMERS MARKET, AND ONE OF OUR KEY NUTRITION STRATEGY STAKEHOLDERS, PROVIDES AND IMPROVES ACCESS TO FRESH PRODUCE ACROSS THE COMMUNITY. THE TRUCK HALVES ITS PRICES FOR SNAP AND EBT CUSTOMERS. HEALTHY SAVANNAH HAS ALSO PROVIDED TECHNICAL ASSISTANCE AND SUPPORT TO THE FORSYTH FARMERS MARKET IN SECURING A GRANT FOR THEIR 'FOOD FARMACY', A 475,000 GRANT OVER THREE YEARS. HEALTHY SAVANNAH'S WORK IN THE LAST TWO YEARS HAS BEEN SOME OF THE MOST FOCUSED AND SUCCESSFUL AT MOVING PEOPLE OUT OF POVERTY IN OUR THIRTEEN YEARS. THAT IS THE POWER OF OUR 200-MEMBER COALITION, OUR CLOSE RELATIONSHIPS WITH THE LOCAL GOVERNMENT LEADERS, THE FAITH-BASED COMMUNITY, AND RESIDENTS IN OUR PRIORITY POPULATION. WE WILL CONTINUE TO ADVOCATE FOR AND IMPLEMENT POLICY, SYSTEMS, AND ENVIRONMENTAL CHANGE (PSE) TO ACHIEVE A CULTURE OF HEALTH IN THE SAVANNAH-CHATHAM COMMUNITY. |
