THROUGH AN INTEGRATED NETWORK THAT INCLUDES ALL SEGMENTS OF THE HEALTH CARE DELIVERY SYSTEM, PACC ACKNOWLEDGES, QUANTIFIES AND COORDINATES PHYSICIAN CHARITY CARE AND SPREADS THAT CARE MORE EQUITABLY AMONGST PHYSICIANS. PACC MAKES IT EASIER FOR PHYSICIANS WHO ARE ALREADY CARING FOR THE UNINSURED TO BETTER MANAGE AND COORDINATE CARE. PACC CLIENTS WHO ARE AT OR BELOW 200% OF THE POVERTY LEVEL AND WHO ARE UNABLE TO OBTAIN HEALTH INSURANCE MAY QUALIFY FOR THE PROGRAM AND ARE REFERRED BY THE COUNTY, PARTICIPATING HOSPITALS, AND VOLUNTEER PHYSICIANS TO PARTICIPATE IN THE PROGRAM. SINCE INCEPTION IN 2011, PACC HAS COORDINATED CARE FOR 820 PATIENTS WITH 262 ACTIVE PATIENTS AS OF DECEMBER 31, 2015. IN 2015, THE PACC NETWORK DELIVERED AN ESTIMATED $1,175,453 IN DONATED MEDICAL CARE TO PACC PATIENTS. PACC SECURED $389,917 IN PHARMACY ASSISTANCE FOR THE CLIENTS AND PAID $93,755.05 FOR PRESCRIPTION BENEFITS. IN TOTAL FOR 2015, THE PACC PROGRAM PROVIDED $1,659,125 IN MEDICAL CARE AND SERVICES, WHICH DOES NOT INCLUDE UNQUANTIFIABLE BENEFITS SUCH AS NAVIGATION, SUPPORT, AND HEALTH EDUCATION. |
