Activity 2

Home delivered and congregate meals for senior adults in burleigh county. Provided 47,281 congregate meals to 1213 individuals. Provided 33,356 home delivered meals to 445 individuals. This is the organization's largest program. In 2011, 87% of the total congregate meals served were to individuals at or below the poverty level, 48% lived alone, and 10% lived in rural areas. Of the home-delivered meal recipients, 90% were at or below the poverty level, 50% lived alone, and 8% lived in rural areas.

Activity 3

Outreach services to senior adults in burleigh county - approximately 5,210 units of service were provided to 818 senior adults. The outreach program refers and links seniors and their families to community services and programs. In 2011, 99% of the seniors served were at or below the poverty level, 53% lived alone, and 9% lived in rural areas. Two licensed social workers are utilized in the outreach program. Through home visits and phone calls the social workers identify the needs of seniors and connect them to a variety of available services.
