Additional Explanations

Form 990, Part IX

These adjustments have been included on form 990 and will be made on the financial statements on a go forward basis.

FORM 990, PART XI, LINE 9:

Contribution of cass community health foundation's net assets at year end 331,860.

Financial Statement Notes

PART V, LINE 4:

The endowment funds of the organization will be used to carryout the exempt activities of the organization in accordance with donor restrictions, if any.

PART X, LINE 2:

The foundation and cchf are exempt from federal income taxes under section 501(c)(3) of the internal revenue code and exempt from state income taxes under the laws of the state of missouri. Ccdc is a single member limited liability company and is considered a disregarded entity for federal income tax purposes. Accordingly, no provision has been made for federal and state income taxes. The foundation assesses uncertain tax positions on an annual basis. There has not been interest or penalties recognized in either the statement of activities and changes in net assets or in the statement of financial position related to uncertain tax positions. In addition, no tax positions exist for which it is reasonably possible that the total amounts of unrecognized tax benefits will significantly increase or decrease within the next twelve months. The foundation is no longer subject to federal or state income tax examinations by tax authorities before 2013.
