Mission

To work for equitable health care access and outcomes for people who experience injustice at the intersection of health status and identity.

Major Activities

Activity 2
Hiv education and prevention services focus on the key objective of stopping the spread of hiv by: reducing risk for new infections; helping those at highest risk learn their hiv status; and supporting open communication and disclosure. Map offers the following programs: map aidsline provides hiv education, risk assessment and access to needed services through phone and on-line contacts. Pride alive builds a community of men who share responsibility for creating and promoting social norms that support hiv and std risk reduction in gay and bisexual men's communities. Positive link builds a community of men living with hiv who take an active role to reduce the impact of hiv in their community; and who work together to manage and maintain their health. Street outreach is part of an effort to reduce hiv, stds and hepatitis c infections among individuals at high risk due to substance using behavior. Hiv education and prevention service programs provided the following services in 2010: aidsline program responded to 4,127 contacts via phone, online chats, email or drop-in visits. In addition, the aidsline provided hiv-related education to an estimated 57,684 visitors to the aidsline section of the map website; distributed 3,660 pieces of hiv educational material to the general public; and provided basic hiv education in-person to 4,936 individuals.pride alive staff and program volunteers made 13,000 contacts through outreach and health education activities including distributing safer sex kits and risk reduction information in venues frequented by gay/bisexual men. This program also provided hiv counseling sessions for 289 individuals, and tested 100% of them through its hiv testing and counseling outreach that target gay and bisexual men at locations throughout the twin cities. Positive link events grew to 409 members connecting hiv-positive gay/bisexual men for social and emotional support and hiv educational events. Positive link also conducted individual meetings with 37 gay/bisexual men new to living with hiv to provide education, emotional support and an orientation to hiv care services. Street outreach activities provided 2,645 at-risk individuals with hiv risk reduction information through harm reduction, outreach, and community education services and events. This program also provided individual outreach and education intervention, including syringe exchange, in 2,590 syringe exchange meetings designed to reduce hiv and hepatitis c risk from injecting drug use, or due to other substance use risk behaviors.
Activity 3
Emergency assistance program targets very low-income clients (those living up to 175% of federal poverty guidelines) living with hiv, and provides short-term assistance with unanticipated housing, utilities, food and medical expenses. In fy 2010, map provided assistance to 1,368 low-income individuals throughout minnesota.

CHEMICAL HEALTH SERVICES SERVED 96 HIV POSITIVE INDIVIDUALS IN 2010 BY PROVIDING SERVICES THROUGH RULE 25 TREATMENT LINKAGE CASE MANAGEMENT (TLCM). CLEAR (CHOOSING LIFE! EMPOWERMENT! ACTION! RESULTS!), A CLIENT-CENTERED PROGRAM THAT DELIVERS COGNITIVE-BASED, BEHAVIORAL TECHNIQUES TO HIV-POSITIVE OR HIGH-RISK-NEGATIVE INDIVIDUALS, PROVIDED COUNSELING SESSIONS FOR UP TO 27 INDIVIDUALS WHO WERE HIV-POSITIVE RESULTING IN MEANINGFUL BEHAVIOR CHANGES. LAUNCHED IN DECEMBER 2010, CLEAR PROVIDED INDIVIDUAL COUNSELING TO 6 PARTICIPANTS IN THAT MONTH ALONE, AND HAS A GOAL OF SERVING 45 PARTICIPANTS IN THE FULL 12-MONTH CONTRACT YEAR.BENEFITS COUNSELING PROGRAM PROVIDED 390 INDIVIDUAL CONSULTATIONS TO PEOPLE LIVING WITH HIV AND PROVIDED 368 CONSULTATIONS TO PROVIDERS SUCH AS CASE MANAGERS AND OTHER PROFESSIONS TO HELP THEM MEET THEIR CLIENTS' NEEDS DIRECTLY AND MORE EFFECTIVELY.
