Form 990, Part VI, Section B, Line 11

A copy of the form 990 was reviewed by management, then provided to the board of directors for approval prior to filing. Approval of the form 990 was set as an action item on the agenda. Upon approval, the accounting firm was notified and the final 990 was printed for their signature.

Form 990, Part VI, Section B, Line 12C

Members must avoid conflict of interest with respect to their fiduciary and financial responsibility. A) members are required to disclose annual interests that could give rise to conflict. B) the board will regularly and consistently monitor and enforce compliance with board members' code of conduct by having a standing agenda item regarding any conflict(s) of interest. C) there will be no self-dealing or business by a member with the organization. Members will annually disclose their involvements with other organizations, with vendors, or any associations that might be or might reasonably be seen as being a conflict. D) when the board is to decide upon an issue, about which a member has an unavoidable conflict of interest, that member shall absent herself or himself without comment from not only the vote, but also from the deliberation. E) board members will not use their board position to obtain employment in the organization for themselves, family members, or close associates. Should a board member apply for employment, he or she must first resign from the board. F) the conflict of interest form(s) are reviewed, minimally, once a year and updated as necessary.

There is a process the board follows prior to establishing a ceo compensation package. The process was last undertaken in 2014: 1. Compensation survey or study that may include comparison of form 990 of other organizations 2. Approval by the board of directors 3. Written confirmation of agreement. Compensation is determined for other officers or key employees using one or more of the following three sources: 1. The mn council of non-profits salary survey is the primary method to determine or estimate a key employee's wages per position; this is updated yearly and facts are gathered through yearly surveys. 2. A secondary resource or confirmation is the bureau of labor statistics. 3. A third resources for confirmation only would be free on-line salary sites, such as salary.com. 4. The fourth resource to determine compensation is the use of an independent consultant who reviews the job descriptions and data comparisons. This process was last undertaken in 2011. This process includes review of comparability data, and documentation of the decision at the time it was made, but does not include review and approval by independent persons.

FOR SOME INDIVIDUALS ADULT FOSTER CARE IS A STEPPING STONE TO GAINING SKILLS THAT COULD HELP THEM MOVE TO A MORE INDEPENDENT LIVING ARRANGEMENT. FOR OTHERS IT PROVIDES THE SUPPORT AND CARES THEY REQUIRE TO LIVE A FULFILLING LIFE. THE OCCUPANCY LEVEL IS AT 99% IN OUR 17 HOMES AND THERE CONTINUES TO BE A GREAT NEED FOR THIS SERVICE IN THE COMMUNITY.

THE PROGRAM ALSO PROVIDES CAREGIVERS ASSISTANCE BY GIVING THEM RESPITE BREAKS FROM THE DEMANDING CARE THEY PROVIDE. CURRENTLY WE SERVE 22 INDIVIDUALS IN OUR ADULT DAY PROGRAM. THE SUPPORTIVE SERVICES PROVIDE ASSISTANCE TO INDIVIDUALS IN THEIR OWN LIVING ENVIRONMENT OR IN THE COMMUNITY. SERVICES MAY INCLUDE SUPERVISION FOR THEIR DAILY LIVING ROUTINES, ASSISTANCE WITH ACCESSING COMMUNITY EDUCATION RESOURCES, MANAGING FINANCES, GROCERY SHOPPING, COMMUNITY EDUCATION CLASSES, ASSISTING WITH SEEKING EMPLOYMENT, SOCIAL EVENTS AND BECOMING AN ACTIVE MEMBER OF THE COMMUNITY. CURRENTLY WE SERVE 5 INDIVIDUALS IN THE SUPPORTIVE SERVICE PROGRAM. BOTH SERVICES CONTINUE TO BE IMPORTANT RESOURCES IN THE COMMUNITY FOR THOSE WITH COGNITIVE/ PHYSICAL DISABILITIES AND MEMORY LOSS.

THIS IS DONE THROUGH THE COMPLETION OF A BEHAVIORAL FUNCTIONAL ASSESSMENT AND THE DEVELOPMENT OF A BEHAVIOR MODIFICATION PROGRAM. THE AMOUNT OF TIME A CONSULTANT IS INVOLVED IN A CASE DEPENDS ON HOW WELL A PERSON RESPONDS TO A BEHAVIOR MODIFICATION PROGRAM. OVER THE PAST YEAR, OPPORTUNITY MATTERS PROVIDED BEHAVIOR CONSULTING TO 12 INDIVIDUALS. THE NEED FOR BEHAVIOR CONSULTING SERVICE IS INCREASING AS THE PROFESSION CHANGES THE WAYS SUPPORTS ARE PROVIDED TO INDIVIDUALS WITH DISABILITIES.

OPPORTUNITY MATTERS AND THE PARTICIPANTS WORK TOGETHER TO PROMOTE THE PROGRAM IN ORDER TO SEEK OUT NEW PARTICIPANTS WHO WOULD FIND THE PROGRAM BENEFICIAL TO THEIR LIFE.
