Residential services:opportunity partners began offering residential services to people with disabilities in the early 1970s, assisting people to live as independently as possible while providing staff support as needed. In 1983, the organization launched its semi-independent living services (sils) program to help people transition from larger facilities into community-based housing and encouraging people who did not require 24-hour supervision to live as independently as possibly in their own homes with our support. In 1986, opportunity partners opened its first house, jorgensen, which is located in minneapolis. The organization currently provides services to more than 200 people with disabilities in their own homes and also provides 24-hour staff support to another 90 individuals living at 21 community residences (four to six residents live in each home).in 2011, after extensive research and input from individuals, families and referral sources, opportunity partners launched homebase, providing semi-independent living in comfortable, convenient apartment settings. The program promotes the highest level of independence of each person by offering either single or shared apartments with access to nearby support staff. Opportunity partners opened homebase in hopkins in 2011 and expanded the program to coon rapids in 2012, with a goal to serve 50 people within the next 3-4 years.people served in opportunity partners' residential programs benefit from opportunities to get involved in their community and make lasting friendships. Residential services are currently offered in hennepin, anoka and sherburne counties, serving 332 individuals in all. Satisfaction surveys and licensing reviews promote opportunity partners' attention to quality in services and property management.2012 highlights:-opened second homebase apartment in coon rapids (hopkins opened in 2011)-served 234 people through in-home services (sils)-served 11 people in homebase-served 87 people in opportunity partners group homes-new semi-independent group home, mcnamara, was added.-customer satisfaction rating of 99.3% was achieved by the combined residential and community living programs-98% of survey respondents strongly agree or agree that staff keep them informed-97% of survey respondents strongly agree or agree that opportunity partners provides frequent community experiences.
