Mission

Community care demonstrates and manages innovative, flexible community-based programs to care for at-risk adults in order to enhance their quality of life and optimize the allocation of community resources.

We are a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible. Our medicare/medicaid long-term care programs serve the needs of older adults and adults with disabilities. We coordinate and deliver a full range of supportive services that help them live safely, confidently and with dignity. More than 10,000 wisconsinites trust in us to provide the quality support needed to help them reach their potential in health, happiness and independence. We are wisconsin-based and have local offices supporting each of the 15 counties we serve.

The cfo, finance committee and board of directors review form 990 prior to filing. The board of directors receives a presentation from the principal with the audit firm preparing the return.

Form 990, Part VI, Section B, Line 12C

Community care requires the members of the board of directors to disclose any potential conflicts of interest annually and whenever a new issue arises. Should the organization become aware of any unreported potential conflicts, the concerns are raised with the individual to determine the existence of any such issue. The disclosures are reviewed by the compliance officer and the board chairperson to determine if any mitigation is required. The board of directors is provided with a summary of member-disclosed conflicts of interest annually and any time the information is changed. Additionally, the compliance officer is aware of the meeting agendas and ensures that any potential conflicts of interest are known and addressed prior to any meeting. Should a conflict be disclosed or identified during a meeting the board chairperson or compliance officer will implement the necessary measures to mitigate the concern. Management and individuals with spending authority are required to disclose potential conflicts of interest annually and whenever new issues are identified. Should the organization become aware of any unreported potential conflicts, the concerns are raised with the individual to determine the existence of any such issue. These disclosures are reviewed by the compliance officer and the chief executive officer.

Form 990, Part VI, Section B, Line 15

The following describes how community care inc. Determines its salary and wages. Community care, inc. Engaged the services of a consulting firm to provide salary range suggestions for top personnel and for managers in the human resources department. In 2021, the consulting firm analyzed all positions within a defined scope and closely reviewed the associated job descriptions. Initial compensation is based on total years of relevant experience; annual merit increases are based on the individual's performance evaluation, their compa ratio, and an organizational merit budget. Each performance evaluation is reviewed and approved by the executive managing the administrator or director completing the evaluation. The human resource officer then reviews the performance evaluations. Salaries of managers in the human resource department are included in a market study to avoid any perception of a potential conflict of interest, as the department sets the ranges for all other positions in the organization.

FORM 990, PART I, LINE 1, DESCRIPTION OF ORGANIZATION MISSION CONT.:

Community care is the exclusive organization in wisconsin to offer each of the three long-term care programs of program of all-inclusive care for the elderly (pace), family care and partnership. Our family care medicaid program for frail elders and adults with intellectual, developmental or physical disabilities offers the flexibility, expertise and service options to help members live as independently as possible for as long as possible at home or in the community. The family care program offers the advantages of having a team of professionals guide members. Our members work with their team to develop a cost-effective plan that includes natural supports and paid services from our extensive network of service providers. We are able to tailor services to our members individual needs, circumstances and preferences. The community care pace program is the only pace plan in the state of wisconsin. Pace offers a benefit package that combines medical care, long-term care and prescription drugs. Pace helps frail and disabled individuals age 55 and older live as independently as possible for as long as possible within the community. Community care's family care partnership program for frail elders and adults with physical, intellectual or developmental disabilities provides comprehensive medical and long-term care.

Cci provides representative payee services for some of its members. The custodial accounts are member collective funds.

PART X, LINE 2:

The organization follows the provisions of asc topic 740, income taxes. As required by the uncertain tax position guidance in asc topic 740, the organization recognizes the financial statement benefit of a tax position only after determining that the relevant tax authority would more likely than not sustain the position following an audit. For positions that meet the more-likely-than-not threshold, the amount recognized in the consolidated financial statements is the largest benefit that has a greater than 50 percent likelihood of being realized upon ultimate settlement with the relevant tax authority. There were no significant matters determined by management to be unrecognized tax benefits taken or expected to be taken in a tax return that have been recorded on the organization's consolidated financial statements for the years ended december 31, 2021 and 2020.

WE ARE WISCONSIN-BASED AND HAVE LOCAL OFFICES SUPPORTING EACH OF THE 15 COUNTIES WE SERVE.OUR FAMILY CARE MEDICAID PROGRAM FOR FRAIL ELDERS AND ADULTS WITH INTELLECTUAL, DEVELOPMENTAL OR PHYSICAL DISABILITIES OFFERS THE FLEXIBILITY, EXPERTISE AND SERVICE OPTIONS TO HELP MEMBERS LIVE AS LONG AS POSSIBLE AT HOME OR IN THE COMMUNITY. THE FAMILY CARE PROGRAM OFFERS THE ADVANTAGES OF HAVING A TEAM OF PROFESSIONALS GUIDE MEMBERS. OUR MEMBERS WORK WITH THEIR TEAM TO DEVELOP A COST-EFFECTIVE PLAN THAT INCLUDES NATURAL SUPPORTS AND PAID SERVICES FROM OUR EXTENSIVE NETWORK OF SERVICE PROVIDERS.
