| IRS990/Desc | 0 | Covenant Community Care uses a patient-centered medical home service delivery model. We offer comprehensive primary medical, dental, and behavioral health care for people of all ages, regardless of their ability to pay. Our six clinical sites are spread across Metro Detroit in an effort to make our services accessible to a population with significant financial, health, and transportation challenges. Chronic conditions such as diabetes, hypertension, depression, asthma, heart disease, HIV, and many others are managed by a multi-disciplinary team of providers, nurses, social workers, benefit coordinators, and volunteers. All sites also have a Behavioral Health Coordinator that functions to integrate the mental health services and lifestyle education needed by each patient to identify and help remedy obstacles to compliance, working in close association with community mental health agencies. Covenant's sixth site Newton is a school-based clinic that provides critical health services year-round to the children and families of a Northwest Detroit school and the surrounding community. In 2015, Covenant served 11,101 patients with 31,464 visits across our six sites. Of these patients, 23% were without health insurance, creating great obstacles to successful healthy living and the management of their chronic diseases. Our commitment to seek out, heal and strengthen "those who need it most" ensures that our percentage of uninsured patients remains substantial as we continue to grow. This can present a challenge as the sometimes transient and under-resourced characteristics of this group make medical compliance and follow up, attainment of health goals, and access to services a challenging goal. For this reason, we are always looking for new partners, like community mental health agencies and other social service organizations, and developing new programs, such as our Homeless Outreach Team, to help us improve how we fulfill our mission of "showing and sharing the love of God, as seen in the good news of Jesus Christ, by providing integrated, affordable, and quality health care to those who need it most."Covenant's quality measures continued to indicate success in 2015: we had a 90% childhood immunization rate, 63% adult weight screening and follow-up rate, a 67% cervical cancer screening rate, and a 31% colorectal screening rate all rates higher than the Michigan state average for health centers. We're also proud to say that our cost per patient in 2015 at $563 was over $250 less than the state health center average. |

More extensive services are performed by a rotating dentist and also are referred out to Michigan or Moross. The Covenant School-Based Dental Team (SBDT) provides dental care to children aged 0-18 at Head-start, elementary, middle and high schools across Detroit as well as Covenant Newton, a school-based clinic in Northwest Detroit. The SBDT provides dental cleanings, x-rays, fluoride, sealants, and exams. If needed, students receive referrals for any additional care.Covenant Community Care offers oral health services at all clinical sites and also through a mobile, school-based dental team. In 2015, we served 8,411 dental patients through 22,843 visits. In 2015, Covenant staff facilitated 124 patient education visits and had 11.4 full time equivalent staff members dedicated to enabling services.Covenant has a licensed chauffer to provide transportation assistance for patients to and from our clinics. Patients are informed of this service while scheduling their appointments and, if needed, arrangements are made at that time. We also employ Benefits Coordinators at each of our sites to assist patients with identifying eligibility status and enrolling in public health plans. All of our services are provided on a bilingual basis, in English and Spanish. Covenant has bilingual staff members at each of our sites and also provides translation services to support patient/provider/staff interactions. All forms, patient satisfaction surveys, educational materials, signage, waiting room reading material, and other administrative materials are produced in Spanish and English. Our Homeless Outreach team provides care to about 50 people per week where they are at (under highway overpasses, in broken down trailers, in abandoned buildings, and in hidden urban encampments), works with them to navigate the present day healthcare system, and dispels the obstacles of anxiety, fear and mistrust. The goal is to meet and treat people on the street, build relationship, eventually get them into our clinics to see Covenant providers in order to establish a primary care relationship, and simultaneously work with partners including a podiatrist, wound-care specialist, and an ophthalmologist in order to care for more acute need. |

Our primary medical and behavioral health care services are provided through an integrated delivery model. To accomplish this, all patients are required to complete a one-page "Today's Visit" tool that identifies recent symptoms of depression. All of our medical providers receive training to ensure that they are able to effectively identify and successfully refer patients who need behavioral health consultations. In June of 2015, Covenant piloted substance abuse screening at our Moross clinic. Similar to the depression process, the pilot involved a modification of the Today's Visit form as the initial screening tool. The integration of behavioral health screenings into every patient encounter at Covenant improves service delivery and outcomes in several ways. By proactively screening our patients, we identify behavioral health needs earlier in their progression, allowing for early intervention by our Behavioral Health Counselor. We also believe that by making behavioral health screening a routine practice, it helps to remove the stigma often associated with receiving behavioral health services. The training provided to our medical providers and support staff enables our employees to effectively assess when a full behavioral consultation is warranted. This is important, because through this practice, our staff can act as a safety net to catch patients who may need a consultation even if they do not self-identify as needing behavioral health care on the Today's Visit tool. |
