Form 990, Part VI, Line 6: Explanation of Classes of Members or Shareholder

Membership is open to any family members and individuals served by the organization, as well as other interested persons, advocates and members of the public. Nominal dues ($15 individual, $25 family).

Form 990, Part VI, Line 11B: Form 990 Review Process

Management participated directly in the completion of responses to questions/items in Form 990, plus reviewed completed documents in detail before submission. Copies of the final Form 990 will be reviewed by the Audit/Finance committee and made available to all Board members for their individual review before filing with the IRS. Review of the audit and 990 are also a Board Agenda item.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Annual Board Conflict of Interest disclosure form is provided to all continuing Board members in January of each year and to all new members when elected, plus is included as Board Agenda item for first regular Board meeting of the year, and for any subsequent meeting Agendas until all members have signed. If topic-specific conflicts appear during the year, members disclose and excuse themselves from votes or other related action on those items. If questionable whether such a conflict or appearance of conflict exists, members have raised the issue of the potential conflict for consideration by the Executive Committee and/or full Board, as appropriate and a determination is made.

Form 990, Part VI, Line 15B: Compensation Review and Approval Process for Officers and Key Employees

The process for determining compensation changes for the CEO/Executive Director is conducted by the Board Executive Committee, as empowered annually by the full Board to act on their behalf, and includes a review of performance against Board-adopted objectives; a review of recent market data for similarly qualified persons with comparable job functions in a similar organization. Such CEO reviews/adjustments correspond to the timing of agency-wide pay reviews/adjustments for all staff. This review and any resulting compensation decisions are documented by the Board Executive Committee for the record and are routinely audited.

Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: Community Services include After School/Transition (high school students - 19 served), Summer Camps, Respite (30 served, 7600 of service hours in 2016) and Community-Based Work Crews in local industries (6 served on the remaining crew not lost as of 2012 to the declining economy).Community Case Management includes United Way's Economic Assistance Planning for families impacted by job loss due to the economy (79 more were referred and 39 eligible families added in 2016). Case management for 3 additional families was provided October through December 2016 through United Way's new No Place to Call Home initiative. These services grew from the successful conclusion of three years Disaster Recovery Case Management serving 915 recovering families for United Way.

Part X : FIN48 Footnote

The Organization has adopted the provisions of FASB ASC 740-10 (FASB Interpretation No. 48), Accounting for Income Taxes, effective January 1, 2009. The Organization does not believe it has any unrecognized tax benefits or tax liabilities (tax positions) for either Federal or State taxing authorities that require disclosure in accordance with FASB ASC 740-10. The Organization continually monitors and evaluates expiring statutes of limitations, audits, proposed settlements, changes in tax law and new authoritative rulings, as determined relative to the Organizations operations. The Organizations management believes it is no longer subject to income tax examinations prior to 2010.

IRS990/Desc0ACCESS Johnson County Public Transportation provided 85,670 local passenger trips during 2016, and added 460 new first-time registered passengers, in addition to 'walk-ons' boarding at bus stops. Of these trips, 39,891 were on the fixed route service which had increased frequency in 2012 through a system redesign made possible by the Jobs Access/Reverse Commute grants from INDOT and IndyGo/Indianapolis Public Transit Corporation, 2016 was the second. 16,993 different individual registered passengers have used Access since its onset, plus an unknown unregistered number of bus stop walk-ons. In addition, Access dispatched volunteer drivers' trips for Veterans to the VA hospital, coordinated public transit services for 3-county service areas including purchased services from local providers, plus helped facilitate networking of central Indiana regional rural transit services with Indianapolis transit system and Central Indiana Regional Transit Authority. To absorb the losses from the successful conclusion of several temporary (2008-2014) Federal 'special projects' grants, a phased-in Transit service redesign and system restructuring in 2014 focused on keeping critical services operational with minimal disruption to passengers, and with no employee layoffs. 2015 was the first full year of implementation.

There were 22 new job placements made during 2016. One deaf individual is now training for a management position and another 'graduated' from our services needing no further assistance. In 2016 Gateway also became a Social Security Ticket to Work Employment Network.
