Form 990, Part VI, Line 6: Explanation of Classes of Members or Shareholder

Membership is open to any family members and individuals served by the organization, as well as other interested persons, advocates and members of the public. Nominal dues ($15 individual, $25 family).

Form 990, Part VI, Line 11B: Form 990 Review Process

Management participated directly in the completion of responses to questions/items in Form 990, plus reviewed completed documents in detail before submission. Copies of the final Form 990 will be reviewed by the Audit/Finance committee and made available to all Board members for their individual review before filing with the IRS. Review of the audit and 990 are also a Board Agenda item.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Annual Board Conflict of Interest disclosure form is provided to all continuing Board members in January of each year and to all new members when elected, plus is included as Board Agenda item for first regular Board meeting of the year, and for any subsequent meeting Agendas until all members have signed. If topic-specific conflicts appear during the year, members disclose and excuse themselves from votes or other related action on those items. If questionable whether such a conflict or appearance of conflict exists, members have raised the issue of the potential conflict for consideration by the Executive Committee and/or full Board, as appropriate and a determination is made.

Form 990, Part VI, Line 15B: Compensation Review and Approval Process for Officers and Key Employees

The process for determining compensation changes for the CEO/Executive Director is conducted by the Board Executive Committee, as empowered annually by the full Board to act on their behalf, and includes a review of performance against Board-adopted objectives; a review of recent market data for similarly qualified persons with comparable job functions in a similar organization. Such CEO reviews/adjustments correspond to the timing of agency-wide pay reviews/adjustments for all staff. This review and any resulting compensation decisions are documented by the Board Executive Committee for the record and are routinely audited.

Part X : FIN48 Footnote

The Organization is exempt from federal and state income taxes on its related activities under Internal Revenue Service Code Section 501(c)(3). Accordingly, no provision for federal and state income taxes has been made.The Organization files the required federal and state information returns. Whenever tax returns are filed, the filing organization must evaluate the merits of its tax positions and determine if they will be ultimately sustained. Those tax positions for the Organization include maintaining their tax-exempt status and the taxability of any unrelated business income. The Organization believes these positions are sustainable. Although the Organization has not incurred any interest and penalties associated with these positions, it is their policy to expense them in the statement of activities.

IRS990/Desc0ACCESS Johnson County Public Transportation provided 78,454 local passenger trips during 2019, and added 483 new first-time registered passengers, in addition to 'walk-ons' boarding at bus stops. Of these trips, 44,572 were on the fixed route service which had increased frequency in 2012 through a system redesign made possible by the Jobs Access/Reverse Commute grants from INDOT and IndyGo/Indianapolis Public Transit Corporation. 18,475 different individual registered passengers have used Access since its onset, plus an unknown unregistered number of bus stop walk-ons. In addition, Access dispatched volunteer drivers' trips for Veterans to the VA hospital, coordinated public transit services for 3-county service areas including purchased services from local providers, plus helped facilitate networking of central Indiana regional rural transit services with the Indianapolis transit system and Central Indiana Regional Transit Authority. To absorb the losses from the successful conclusion of several temporary (2008-2014) Federal 'special projects' grants, a phased-in Transit service redesign and system restructuring in 2014 focused on keeping the most-used and most critical services operational with minimal disruption to passengers, and with no employee layoffs.

IRS990/ProgSrvcAccomActy2Grp/Desc0Disability & Community Services - Community Employment Services served 45 new referrals in 2019 and assisted 122 individuals with disabilities, including 14 who are deaf or hard of hearing, with their job search, training, and ongoing employment/career support, including sustaining average individual job retention success for Supported Employment. 10 individuals celebrated continuous successful community employment of 10 to 30+ years, ranging up to the current highest individual continuous job retention of 33 years despite fluctuating economic conditions over those years. 28 of the individuals successfully holding community jobs received extended career support/follow-along services There were 32 new job placements made during 2019, including 6 of the 14 individuals who received Deaf Services, 4 of whom started full-time benefitted positions and are working their way off Social Security. In 2016 Gateway also became approved as a Social Security Ticket to Work Employment Network, which in 2019 provided ongoing services to 2 individuals in Deaf Employment Services. - Habilitation Services provided in 2019 to individuals with disabilities included service coordination (242); individualized community-based activities with staff support (42); Day Program (65), which also began offering extended hours to families in 2019; scheduled large group events (71) and emphasized meaningful community participation, access to community opportunities and local resources, volunteerism, handling of life events with support from staff, skill development, inclusive recreation/leisure activities and events, and personal enrichment. An average of 26 young adults participated in each monthly Club 31, now in its 7th year; 19 individuals participated in the Franklin College Best Buddies chapter activities and community events; and the 16 members of our local Kiwanis 'Aktion Club' chapter celebrated 24 years of fellowship and inter-club volunteer community service projects, which in 2019 included supporting Riley Childrens Hospital and Salvation Army. - Other Community Services include After School/Transition and Summer Camps with 20 high school students from 5 Johnson County schools served, developing skills for employment, leadership, and community involvement through local partnerships. In 2019, 1 student began successful community employment, 1 moved from her parents home to semi-independent living, and 1 more student graduated from this program and from high school to pursue opportunities of their own choice, which over the years has included a university computer science degree program; an internship at a local hospital; community employment; and further participation in Gateway's habilitation and community support services. Transition and Employment Services working together prepared 2 more students to successfully open Vocational Rehabilitation cases, receiving supported employment services through Gateway. Respite Services provided caregiver support to 31 families in 2019 through 4,944 hours of respite care, supplied by a Gateway trained relief caregiver who the family personally chooses from individuals they already know and trust. 5 additional individuals continue their work as members of a Community-Based Work Crew at NSK. - Community Case Management includes United Way's Economic Assistance Planning focusing on regaining financial stability for families impacted by job loss due to the economy or other disrupting life events. Of the 30 total families served in 2019, 20 were new eligible families added, out of 28 referrals. Case management for 11 additional families was provided in 2019 (including 4 new referrals) as part of a network of local agencies through United Way's new No Place to Call Home initiative, begun in late 2016 to help prevent homelessness in Johnson County, with a similar goal of establishing family financial stability for those at risk.
